Professor Ja-won Koo's research lab specializes in vestibulopreservation and otoneurological disorders, focusing on the pathophysiology of inner ear diseases such as Meniere’s disease, vestibular schwannoma, and endolymphatic hydrops. The lab investigates genetic susceptibility, autoimmunity (particularly anti-type II collagen responses), and the role of inflammatory mediators in hearing and balance disorders. Key research directions include the clinical and molecular characterization of intralabyrinthine schwannomas, the impact of systemic inflammation on cochlear drug uptake, and the development of diagnostic criteria for rare inner ear pathologies like superior canal dehiscence. The ototoxic aminoglycoside antibiotics are essential to treat severe bacterial infections, particularly in neonatal intensive care units. Our results demonstrate that patients may develop vestibular symptoms during their clinical course, and all patients with an enlarged vestibular aqueduct should be cautioned regarding the potential development of vestibular pathology. Moreover, the non-negligible incidence of secondary BPPV mandates positional tests when evaluating EVA patients with vertigo.
To investigate HLA-associated genetic susceptibility to Meniere's disease in relation to type II collagen (CII) autoimmunity status, HLA-DRB1 genotyping and ELISA measurement of anti-CII antibody were performed in 41 Korean patients with Meniere's disease. <b>Objectives:</b> The aim of this study was to delineate the clinical and laboratory features suggestive of intralabyrinthine schwannoma (ILS). <b>Methods:</b> We compared the clinical features of 16 patients with ILS, who had been diagnosed at the Seoul National University Bundang Hospital from 2003 to 2018, with those of 18 patients with symptomatic unilateral intracanalicular schwannoma and randomly selected 20 patients with definite or probable unilateral Meniere's disease (MD). <b>Results:
Comorbid BPPV is a negative prognostic indicator of auditory function in ISSNHL. Concurrent BPPV in ISSNHL suggests combined damage to the utricle and may indicate severe and widespread labyrinthine damage, leading to the poor prognosis.

Among 26 patients with radiologically confirmed EVA, 5 (19.2%) exhibited positional nystagmus compatible with BPPV through Dix-Hallpike and head-roll tests. Hearing loss usually accompanied BPPV attacks, and BPPV was recurrent in three patients. Multiple semicircular canals were frequently involved in each episode, and different canals were also involved in recurrent cases. Canalith repositioning procedures were usually successful without difficulty.
Background and Objectives laser is known to have optimal tissue characteristics for stapes surgery though it has suboptimal optical characteristics. Surgical experience of stapes surgery using laser has not been previously reported in Korea. In patients with CP over 43% (N =23), 2 beated to ipsilesional side, 17 to contralesional side and 4 showed no VIN. Transtympanic resurfacing with bone cement, reinforcing the dehiscent JB to reduce focal turbulent flow, is a simple and effective surgical treatment option in patients with PT due to HJBD.
