Professor Takuya Aoki's research lab focuses on improving the quality and equity of primary care in Japan, with a strong emphasis on patient-centered outcomes. The lab investigates multimorbidity patterns and their impact on medication use, explores patient experience of primary care through validated tools like JPCAT and patient experience scales, and examines social determinants such as social isolation and health literacy that influence care quality. A central theme is enhancing primary care delivery to reduce health disparities and support person-centered, comprehensive care, particularly for older adults and vulnerable populations.

In the present study, we aimed to identify multimorbidity patterns in a Japanese population and investigate whether these patterns have differing effects on polypharmacy and dosage frequency. We found that better patient experience of primary care was strongly associated with ACP discussion. Our findings reinforce the significance of patient experience in primary care as part of quality end-of-life care.
Social isolation was associated with negative patient experience in elderly primary care patients. Raising awareness regarding patient social networks among primary care providers and targeted interventions for socially isolated elderly patients aimed at improving the experience of primary care, especially regarding longitudinality, comprehensiveness, and community orientation, may be warranted.

Using an atmospheric metal-organic chemical vapor deposition system, we passivated GaAs with AlN prior to atomic layer deposition of Al2O3. We developed a concise patient experience scale, which comprises six domains measuring primary care attributes and evaluated its validity and reliability. This scale can be used as a rapid assessment tool reducing the burden of respondents and provide effective information for further quality improvement and practice-based research in the Japanese primary care settings.

Our study revealed that multimorbidity patterns have different effects on the clinically relevant decline in HRQoL for 1 year. These findings can be useful in identifying populations at high risk and with poor clinical outcomes among patients with chronic diseases and multimorbidity for efficient resource allocation.
Our study elucidates differences in the strengths and challenges of primary care between hospital-based practices and community-based office practices, with a focus on patient centeredness. Improving community orientation in hospital-based practices and improving accessibility, including out-of-hours care, in community-based office practices may enhance the quality of primary care and promote standardization of care across settings.
