Peri-implant diseases are known as undesirable conditions that can occur after implant therapy. 症例の概要:患者は74歳の女性で,主訴は下顎全部床義歯の動揺による咀嚼困難であった.下顎は無歯顎であり,顎堤は全体的に高度に吸収していた.上顎の残存歯に対する前処置を行った後,診断用義歯として上下顎全部床義歯を製作した.その後,最終義歯として,下顎に2本のインプラントを支台としたオーバーデンチャーを製作した.
医療や介護の現場で日常的に発生する情報を集めた「リアルワールドデータ」は,近年,臨床研究で用いられる機会が多くなっている.本稿では,2017~2021年度に行われた大阪府後期高齢者歯科健診の約25万人分のデータと,国保データベースシステムの医療・介護データを基とした大規模コホート研究から,欠損歯列が欠損の拡大に与える影響について,筆者がこれまでに得た知見を紹介したい.さらに,臼歯部咬合支持と死亡との関連を評価した結果を踏まえ,Eichnerの分類,Cummerの分類,そして宮地の咬合三角といった咬合支持の観点から,欠損歯列が更なる歯の欠損,そして全身へ与える影響について考察する.
