면역항암제의 도입은 기존 치료법의 한계를 극복하며 비소세포폐암 치료의 새로운 패러다임을 제시하였다. 본 연구는 실제 임상 환경에서 면역항암제 도입 전후 국내 진행성 비소세포폐암 환자의 사망률 변화를 평가하고, 면역항암제 도입 후 진단된 환자군의 주요 예후 요인을 규명하고자 수행되었다. 국가 암 등록, 국가검진, 국민건강보험 청구 및 사망 정보를 환자 단위로 결합한 데이터베이스 K-CURE 암 공공 라이브러리를 활용하여, 2015년부터 2019년까지 진행성 NSCLC로 진단된 환자 76,379명을 대상으로 후향적 코호트 분석을 수행하였다. 환자는 면역항암제의 건강보험 등재 시점을 기준으로 도입 전(pre-IO)과 도입 후(post-IO)의 두 군으로 구분되었다. 1년 사망률은 도입 전 56.1%에서 도입 후 51.6%로 유의하게 감소하였으며(p < 0.0001), Cox 회귀분석 결과 전체 사망위험이 11.3% 감소한 것으로 나타났다(Adjusted HR 0.887, 95% CI 0.
