Among elderly individuals with AO type A2 intertrochanteric fractures, patients treated with bipolar hemiarthroplasty were able to perform early ambulation. However, no significant difference in operation time, amount of postoperative transfusion, clinical results, hospital stay, and radiological failure rate was observed between the bipolar hemiarthroplasty and compression hip screw fixation groups.

목적: Vancouver B2, B3형 대퇴스템 주위골절 치료시 기존의 대퇴스템을 치환한 경우와 유지한 경우의 결과를 분석하였다. 대상 및 방법: 1993년 1월부터 2004년 10월까지 본원에서 수술한 Vancouver B2형 골절 19예와 B3형 골절 8예를 대상으로 하 였다. 기존 스템을 사용한 경우(A군)는 15예, 긴스템으로 치환한 경우(B군)는 12예이었다. 두군 모두 금속판 및 나사못, 케이블등 으로 견고한 내고정을 하였으며, 임상적 평가는 Harris 고관절 점수, 방사선학적 평가는 Beals 및 Tower의 기준을 이용하였다. 결과: Harris 고관절 점수는 A군이 84점, B군이 85점이었다. 80점 이하의 불만족은 A군에서 2예, B군에서 2예 관찰되었다. 방 사선학적 평가는 A군의 대퇴스템 해리 1예와, B군의 골절 불유합 1예가 불량이었고, 나머지는 모두 우수로 판정되었다. 합병증 은 대퇴스템 해리, 불유합, 표재성 감염, 탈구, 비구컵해리가 각각 1예씩 발

Vancouver 분류에 근거한 대퇴삽입물 주위 골절의 치료

=목 적: 인공 고관절 치환술 후 대퇴 스템 주위골절 치료 시 Vancouver 분류에 따른 치료원칙으로 치료한 후 치료결과에 대하여 조사하고자 한다.대상 및 방법: 1981년 5월부터 2003년 2월까지 대퇴 스템 주위에 발생한 골절 35예(34명)를 대상으로 하였다. 평균 연령은 56세(30-83)였다. 치료 결과는 Beals와 Tower의 기준으로 판정하였다.결 과: 인공 고관절 치환술 후 대퇴 스템 주위골절의 전체 빈도는 0.91%였고 Vancouver 분류상 발생빈도는 B1, B2, B3, C, AG, AL의 순서였다. Vancouver 치료원칙에 따른 치료 결과는 27예에서 우수, 5예에서 양호, 3예에서 불량의 결과를 얻었다. 총 6예에서 골절을 일으킨 것으로 추정되는 위험요소(골다공증 4예, 골용해 1예 및 대퇴스템 해리 1예)가 발견 되었다. 골절 치료와 관련된 합병증으로는 골소실 1예, 감염을 동반한 불유합 1예가 있었고, 인공 삽입물 치료와 관련된 합병증으로는 대
