재태주령 32주 이하 미숙아에서 생후 1주 이후 후기 저혈압 및 부신 기능부전과의 관계

목적: 미숙아에서 생후 1주 이후에 생기는 미숙아 후기 저혈압은 드물지 않고 생후 1주 이전과는 다른 원인들이 작용할 수있다. 최근에는 부신 기능부전(adrenal insufficiency, AI)에 의한 급격한 순환부전이 후기 저혈압의 주된 원인으로 생각된다. 이에 본 연구자들은 미숙아 후기 저혈압의 빈도 및 원인을 살펴보고, AI와의 관련성에 대해 알아보고자 한다. 방법: 2009년 1월부터 2011년 4월까지 총 2년 3개월 간 서울대학교병원과 보라매병원에서 출생하여 신생아 중환자실에 입원한 재태 주령 32주 이하의 미숙아 총 244명을 대상으로 후향적 의무기록 고찰을 시행하였다. 이들을 미숙아 후기 저혈압의유무에 따라 산전, 신생아 병력을 비교하였고, 로지스틱 회귀분석을 통해 독립적으로 유의한 위험인자를 구하였다. 이후 저혈압 환아들을 대상으로 hydrocortisone 투여 유무 및 AI유무에따라 각각 두 군으로 나누어 산전 및 신생아 병력, 저혈압의 임상양상을 비교하였다.

소아의 임상 검체 및 건강한 소아의 비인두에서 분리된 페구균의 혈청형 및 페니실린 감수성

목 적 : 폐구균은 건강한 소아의 비인두에서 분리될 수 있는 정상 균총이지만, 다양한 종류의 침습성 감염과 국소 감염을 일으킨다. 최근에 영아 및 어린 소아에서도 침습성 감염에 대한 예방 효과가 우수한 7가의 단백 결합 백신이 개발되었다. 백신의 적절한 사용을 위해서는 백신을 사용하고자 하는 지역에서 분리되는 폐구균의 혈청형에 대한 분석이 필요하다. 방 법 : 2000년 3월부터 2002년 2월까지 2년간 서울대학교 어린이병원, 이화여자대학교 동대문병원, 성균관대학교 강북삼성병원에 내원한 15세 미만의 환아의 임상 검체에서 분리된 폐구균 127 균주와 1997년 11월부터 1998년 10월까지 1-5세 사이의 건강한 소아의 인후 도찰 검체에서 분리된 폐구균 72균주를 대상으로 Quellung reaction을 이용하여 혈청형을 결정하였으며, oxacillin 디스크 확산법을 이용하여 penicillin에 대한 감수성을 검사하였다. 결 과 : 임상 검체에서 폐구균이 분리된 소아의 연령
