목 적:증가된 혈중 및 조직의 plasminogen activator inhibitor type 1 (PAI-1)은 심근 경색증을 포함한 다양한 혈관합병증의 발생과 관련이 있음이 보고되었고 최근에는 이러한 PAI- 1 유전자의 일부 다형성이 PAI-1 활성화와 관련되어 있다고 알려지었으나 아직 한국인 당뇨병의 합병증에 미치는 영향에 대해서는 연구가 미비한 실정이다. 이에 저자들은 PAI-1의 유전적 다양성이 한국인 당뇨병성 신부전 발생에 미치는 영향을 연구하였다. 방 법:경희대학교 의료원에 2형 당뇨병으로 10년 이상 등록된 환자 중 당뇨병 진단 후 20년 이내에 말기신부전증으로 진행한 92명 (남/여:28/64, 평균연령 61.2세)과 정상 신기능을 보이는 당뇨병 대조군 85명 (남/여:24/61, 평균연령 63.3세)을 대상으로 PAI-1 유전자의 다형성 중 promoter -675 부위의 4G/5G 다형성과 promoter -845 부위의 A/G 다형성을 비교하였다. 결 과:
