Professor Hyeok-min Lee's research lab specializes in antimicrobial resistance (AMR) surveillance, molecular epidemiology of bacterial pathogens, and the genetic mechanisms underlying drug resistance. The lab focuses on monitoring and analyzing multidrug-resistant organisms such as *Neisseria gonorrhoeae*, *Staphylococcus aureus*, and *Chlamydia trachomatis*, with particular attention to emerging resistance patterns and plasmid-mediated resistance. Their work contributes to national and global health initiatives, including the GLASS and Kor-GLASS surveillance systems, and emphasizes the clinical implications of genetic mutations in resistance development.

A pathogenic plasmid free C trachomatis variant was identified. Clinicians should be alerted to the possibility of undetected C trachomatis infection caused by such variants and the potential of azithromycin failure in patients with recurrent chlamydial NGU. The occurrence of this variant is rare and should not form the basis for judgment of the performance or usefulness of plasmid based NAATs for C trachomatis detection.
OBJECTIVES: Antimicrobial resistance (AMR) in Neisseria gonorrhoeae is a major concern globally; however, no comprehensive AMR data for gonococcal isolates cultured after 2006 in Korea have been published internationally. The purpose of this study is to determine the correlation between use of antimicrobials, such as fluoroquinolone, cefoxitin, and cefotaxime, and Escherichia coli resistance using a nationwide database. The incidence of penicillinase-producing N. gonorrhoeae declined significantly, but none of the isolates were susceptible to penicillin G. All isolates were susceptible to spectinomycin, in contrast majority were resistant to tetracycline. Inappropriate use of fluoroquinolone was frequent. The minimum inhibitory concentrations of ceftriaxone were within the susceptible range for all isolates, but those of cefixime were slightly higher, and it was 0.5 μg/mL (nonsusceptible) for 1 isolate.
In South Korea, surveillance of antimicrobial drug resistance in Neisseria gonorrhoeae is extremely limited. We describe the emergence and subsequent national spread of N. gonorrhoeae strains with mosaic penA alleles associated with decreased susceptibility and resistance to extended-spectrum cephalosporins. 배경:새로운 내성균이 출현하고 흔해짐에 따라, 전국 규모의 내성율을 조사하는 것은 매우 중요하다. 본 연구에서는 2004년 상반기 전국 12개 대학병원 및 종합병원에 내원한 환자에서 분리된 주요 세균의 항균제 내성율을 분석하였다. 방법:전국 주요 대학병원 및 종합병원 총 12개 기관에서 분리된 주요 세균의 항균제 내성율을 조사하였다. 2004년 4월부터 11월 사이에 임상검체에서 분리된 주요 세균의 항균제 감수성 양상을 각 기관별로 종합하여 환자의 입원 형태에 따라 분석하였다. 결과:주요 원내감염균인 MRSA는 전체적으로 67%, 중환자실 환자에서는 86%에서 분리되었다. The emergence of high-level resistance to ceftriaxone is giving rise to serious concern about absence of effective treatment options to cure gonococcal infections. Determination of the major serogroups is an important step for establishing a vaccine programme and management strategy targeting Neisseria meningitidis. From April 2010 to November 2016, a total of 25 N. meningitidis isolates were collected in South Korea, in collaboration with the Korean Society of Clinical Microbiology.
