Professor Ji Won Lee's research lab specializes in pediatric hematology and oncology, with a focus on optimizing conditioning regimens and therapeutic strategies in hematopoietic stem cell transplantation (HSCT) for children with high-risk cancers such as neuroblastoma and acute lymphoblastic leukemia (ALL). The lab investigates pharmacokinetic optimization through therapeutic drug monitoring (TDM) of agents like busulfan, explores alternatives to total body irradiation (TBI) to reduce long-term toxicity, and examines genetic factors influencing drug response and toxicity in iron chelation therapy. The lab also investigates novel approaches to improve endoscopic management of biliary tract diseases in pediatric patients.

15BACKGROUND: This study investigated the clinical significance of MYCN amplification within high-risk neuroblastoma (NB). METHODS: Medical records of 135 patients who were diagnosed with high-risk NB from 2004 to 2016 were reviewed. Although <i>ALK</i> mutation was rare and no amplification was observed, ALK protein expression was found in a significant number of patients and was correlated with advanced stage and high-risk neuroblastoma. ALK protein expression could be considered as a marker related to the aggressive neuroblastoma, but it was not the independent prognostic factor for the outcome.
The patients who relapsed or had refractory osteosarcoma or who had metastasis at diagnosis or a lower grade of necrosis after neoadjuvant chemotherapy showed poor prognosis. HDCT with ASCT could be an alternative treatment option for these patients.

BACKGROUND: I-metaiodobenzylguanidine (HD-MIBG) treatment into killer immunoglobulin-like receptor (KIR)/HLA-ligand mismatched haploidentical stem cell transplantation (haplo-SCT) in improving the survival of children with neuroblastoma who failed previous tandem autologous SCT. PROCEDURE: If the patient remained progression free with salvage treatment, HD-MIBG treatment (18 mCi/kg) was given prior to reduced-intensity conditioning (cyclophosphamide + fludarabine + antithymocyte globulin).
