아카시 마사이끼 교수의 연구실은 주로 알레르기성 질환, 특히 비IgE 매개 식품유발성 장내 염증성 질환인 FPIES(식품단백질유도장염증증후군)에 중점을 두고 있으며, 일본에서 급증하고 있는 달걀 기반 FPIES의 유병률, 발症 기전, 진단 및 치료 전략에 대한 종합적 연구를 수행하고 있습니다. 또한 아토피성 피부염, 계절성 천식 등 기타 알레르기성 질환의 유병률과 관련된 대규모 인구 연구를 통해 국내 알레르기 질환의 패atters를 규명하고 있습니다. 특히 국제 공식 가이드라인 도입 이후 세계적 추세와 일본 내 특수성을 비교 분석하는 데 주력하고 있습니다.

Food protein-induced enterocolitis syndrome (FPIES) is a type of non-IgE-mediated gastrointestinal food allergy. 日本語版 ISAAC(International Study of Asthma and Allergies in Childhood)調査用紙を用いて全国小・中学生気管支喘息の有症率調査をおこなった.対象者は小学1,2年生および中学2,3年生を各都道府県それぞれ1,200名以上となるように抽出した.小・中学生の喘息期間有症率はそれぞれ13.9%,8.8%であった.小学生の喘息期間有症率は男子で有意に高い結果となった.アレルギー性鼻結膜炎,アトピー性皮膚炎の症状をあわせると,小中学生とも約3分の1がアレルギー疾患を有している結果となった.47都道府県別の喘息期間有症率は小中学生とも最大で約2倍の差が認められたが,ばらつきは少なかった.
食物蛋白誘発胃腸炎(Food protein-induced enterocolitis syndrome:FPIES)は非IgE依存性食物アレルギーの一病型であり,原因食物を摂取した1~4時間後に皮膚症状や呼吸器症状などのIgE依存性反応を伴わない繰り返す嘔吐で特徴づけられている.2017年の国際コンセンサスガイドライン公開により,世界各国でFPIESに関する報告が増えている.海外におけるFPIESの原因食品は,牛乳・米・魚が多く,鶏卵がそれに続いている.一方,日本ではこの数年で鶏卵,特に卵黄によるFPIESの患者が急激に増加しており,現在では原因食品として最も頻度が高い.このため,鶏卵FPIESの発症年齢,予後などの疫学面では日本を中心に解明が進んでいるが,原因抗原・感作経路・嘔吐の機序・免疫学的機序については不明な点が多い.診断の補助となる検査,安全で信頼性の高い食物経口負荷試験や長期管理の方法も確立されていない.今後これらが明らかにされることで,急増する鶏卵FPIES患者数に歯止めがかかることが期待される.
