서울대학교병원에서 활동하는 홍석균 교수의 연구실은 순수 복강경 기증자 간이절제술(Laparoscopic Donor Right Hepatectomy)의 안전성과 시행 가능성에 중점을 두고 있으며, 특히 장기적인 기증자 및 수혜자 결과 분석을 통해 최신 수술 기법의 임상적 유효성을 입증하고자 합니다. 3D 시각화와 ICG 형광 영상 유도 수술 기법을 접목한 정밀한 수술 기술 개발도 핵심 과제로 삼고 있으며, 간세포암이 있는 환자에 대한 수술 기준 전환을 위한 생물학적 마커 기반 평가 기준 도입을 제안하고 있습니다. 이는 국내외에서 가장 많은 수의 복강경 기증자 간이절제술 사례를 축적한 연구 기반의 임상 연구를 지속하고 있습니다.

Purely laparoscopic donor hepatectomy differs from open donor hepatectomy in terms of angle and caudal view. Therefore, surgeons experienced in open donor hepatectomy must gain adequate experience in laparoscopic liver surgery and make adjustments when performing PLDRH.
According to recent international consensus conferences, pure laparoscopic donor hepatectomy (PLDH), particularly right and left hepatectomies, is not yet considered a standard practice because studies remain limited. Thus, we present the outcomes of more than 500 PLDH patients, mostly receiving a right hepatectomy. To our knowledge, this is the largest study to date on PLDH. Data from all living liver donors who underwent PLDH at 5 centers in Korea until June 2018 were retrospectively analyzed.
In conclusion, our data and experience suggest that a continued paradigm shift from a conventional size based criteria to a biological marker based criteria is indicated when evaluating LDLT candidates with HCC.
PLDRH is feasible when performed at an experienced living donor liver transplantation center. Further studies on long-term recipient outcomes including biliary complications are needed to confirm the safety.

The aim of this study was to compare the clinical outcome and cost-effectiveness of preoperative biliary drainage (BD) methods in periampullary cancer, and to suggest guidelines for selecting the appropriate preoperative BD method.
