타케시 키파라 교수의 연구실은 내과 및 심장혈관 질환 분야에서 약물이석 스텐트의 장기적 안정성과 합병증, 특히 스텐트 혈전증의 발생 메커니즘을 중심으로 연구를 진행하고 있습니다. 특히 1세대 약물이석 스텐트(시롤리무스-이매르 스텐트)의 장기적 합병증인 매우 늦은 시점의 스텐트 혈전증과 목표부위 재혈관화 위험을 분석하며, 항혈소판 치료의 지속 여부가 임상 결과에 미치는 영향을 규명하고 있습니다. 이는 향후 보다 안정적이고 지속 가능한 심장 스텐트 개발에 기여할 수 있는 기초 자료를 제공합니다.

Discontinuation of both thienopyridine and aspirin, but not discontinuation of thienopyridine therapy only, was associated with an increased risk of stent thrombosis. Landmark analysis did not suggest an apparent clinical benefit of thienopyridine use beyond 6 months after sirolimus-eluting stent implantation.

Late adverse events such as very late ST and late target lesion revascularization are continuous hazards, lasting at least up to 5 years after implantation of the first-generation drug-eluting stents (sirolimus-eluting stents), which should be the targets for developing improved coronary stents.
ST timing-dependent differences in baseline demographic features, Thrombolysis in Myocardial Infarction flow grade, and mortality rate suggest possible differences in the predominant pathophysiological mechanisms of ST according to timing after sirolimus-eluting stent implantation.
To understand remodeling of human coronary arteries undergoing coronary angioplasty or atherectomy, serial intravascular ultrasonographic examinations were performed at preintervention and postintervention examinations and at 24 hours, 1 month, and 6 months. Complete serial data were obtained in 61 lesions (balloon angioplasty, 35 lesions; directional atherectomy, 26 lesions). Lumen area improved from 6.81+/-2.24 mm2 after intervention to 8.22+/-2.79 mm2 at 1 month (P=.0001) and decreased to 4.8
The efficacy and safety of coronary stenting seemed to be clinically sustained at 7 to 11 years of follow-up. However, late luminal renarrowing beyond 4 years was common, which demonstrates the need for further follow-up.
