宮本進教授の研究室は、脳血管疾患、特に煙道様脳症(モヤモヤ病)をはじめとする脳卒中の病態解明と治療戦略の確立を主眼としています。特に、再発出血予防を目的とした外科的血流再建術(EC-ICバイパス)の有効性を示す国際的臨床試験「日本成人モヤモヤ病試験」の立案・実施に貢献しており、脳血流動態の改善と長期的予後との関連を解明しています。また、後頭底部循環の障害に伴う視覚障害のメカニズムや、脳動静脈奇形の治療戦略の再考についても革新的な知見を提供しています。

Eighty-two cases of cerebrovascular moyamoya disease were studied by cerebral angiography and computerized tomography. Occlusive lesions were demonstrated not only in the anterior circulation but also in the posterior circulation, and they were associated with the development of an abnormal vascular network (moyamoya vessels). Palliative treatments cannot prevent bleeding and may even worsen the posttreatment course compared with the natural history of cerebral AVMs. A more conservative indication is required in recommending palliative treatment alone.
The involvement of the posterior circulation in moyamoya disease was studied in 178 patients. Forty-three had several types of disturbance such as visual field defect, decreased visual acuity, episodes of blindness, and scintillating scotomata.
