연구실 소개
신철민 교수의 연구실은 위암의 발병 기전과 위험도 평가를 중심으로, 헬리코박터 파일로리 감염, 유전적 소인, 에피제네틱스 변화(특히 DNA 메틸화)의 상호작용을 체계적으로 규명하고 있습니다. 특히 가족력, 음주 습관, 대사 이상 등 환경적·유전적 요인이 위암 발생에 미치는 영향을 다각도로 분석하며, 조기 진단 및 예방 전략 개발에 기여하고 있습니다. 연구는 임상적 적용이 가능한 생물학적 지표 발굴과 함께, 치료 후 변화하는 분자적 변화를 시간 경과에 따라 관찰하는 종합적 접근을 취하고 있습니다.

A dose-response association between alcohol intake and the risk for gastric cancer was not observed. However, ALDH2 polymorphisms were found to modify the susceptibility to the development of gastric cancer associated with alcohol intake, especially in case of ALDH2 *1/*2 genotype. The findings suggest an alcohol-ALDH2 genotype interaction in gastric carcinogenesis.
Changes of DNA methylation in gastric mucosae after eradication of Helicobacter pylori have not been clarified yet. In Korean adults, obesity contributes to the incidence of colorectal cancer with a sex difference. Nonobese but metabolically unhealthy men are considered to be a high-risk group for colorectal cancer, but obesity itself is more important in colorectal carcinogenesis. Any one of biopsy-based tests or serology was found to be excellent for identifying current H. pylori infection among individuals without AG or IM and/or younger patients (<40 years). However, a combination of at least two tests is necessary in the clinical setting of AG or IM.
Overall, <i>H. pylori</i> eradication provides significant improvement of symptoms in functional dyspepsia patients regardless of <i>H. pylori</i> prevalence. However, in the analysis of studies from Asia, the eradication did not significantly improve dyspeptic symptoms. In this region, eradication for dyspepsia can be individualized.
Genome-wide methylation profiles showed a number of genes differentially methylated by H. pylori infection. Methylation profiles in noncancerous gastric mucosae from the patients with gastric cancer can be affected by H. pylori-induced gastritis. Differentially methylated CpG sites in this study needs to be validated in a larger population using quantitative methylation-specific PCR method.
The intrafamilial aggregation of GC might be associated with environmental factors during childhood or TGFB1-509 genetic polymorphism, or possibly H. pylori virulence. These factors may promote IM and development of intestinal-type GC.
