연구실 소개
김은규 교수의 연구실은 암의 이질성과 치료 내성 메커니즘을 해소하기 위해 영상유전자학(radiomics)과 분자생물학을 융합한 연구를 수행하고 있습니다. 특히 <sup>18</sup>F-FDG PET/CT 영상에서 유도하는 텍스처 특징을 통해 종양 내 이질성(intratumoral heterogeneity)을 정량화하고, 이와 연관된 분자 표지자(예: Ki-67, HER2-STAT3-survivin 경로)를 규명하여 치료 반응 예측 및 예후 평가에 기여하고자 합니다. 또한 신재생 능력과 약물 내성의 기전을 밝히는 데 초점을 맞춘 임상 기반의 분자 기전 연구를 지속하고 있습니다.

Our study showed that CTL and NK cell-associated gene expression and protein levels differ significantly according to TIL levels and that CTL-NK score and distribution of TILs within tumors have a prognostic value. These findings emphasize the importance of CTLs and NK cells as well as the spatial uniformity of TIL infiltration in clinical outcome of TNBC patients, providing valuable insights for refining prognostic assessments and guiding immunotherapeutic strategies.

While IMF and PR incisions yielded similar aesthetic outcomes post-radiation, IMF incisions showed trends toward better symmetry, especially at higher radiation doses. These findings support the IMF incision as a favorable choice in NSM with DTI reconstruction followed by PMRT, though patient anatomy and preferences remain critical for surgical planning.
Given the prognostic implications of HER2 discordance which primarily involves zero-to-low conversion and the therapeutic benefits of newly developed antibody-drug conjugates in HER2-low breast cancers, HER2 status should be re-evaluated in surgical resection specimens following NAC.
By integrating breast cancer pathology information into the OMOP-CDM, this study facilitates the unified analysis of clinical and pathological data for retrospective oncology research. Moreover, the proposed NLP-based pathology report standardization framework for integrating into the OMOP-CDM can be readily adopted by other institutions as the use of OMOP-CDM continues to expand in cancer research.
Elevated preoperative NLR and PLR are associated with an increased risk of capsular contracture, particularly in patients receiving radiotherapy. These findings suggest that systemic inflammation, which is potentially exacerbated by radiation-induced fibrosis, may play a synergistic role in the development of capsular contracture. These markers may aid in preoperative risk stratification and support more personalized surgical planning.
