연구실 소개
이휘재 교수의 연구실은 심정지, 패혈성 쇼크, 응급실에서의 감염성 질환 등 중증 질환의 조기 진단과 예후 예측, 치료 전략 개선을 핵심으로 삼고 있습니다. 특히 C-반응성단백질, 프로칼цит론과 같은 생체마커의 예후적 가치, 심 cardiopulmonary resuscitation(CPR)의 질적 요법 비교, 뇌 손상 억제를 위한 저체온요법의 작용 기전 등 응급의학 및 감염병 분야의 임상적 실마리를 제공하는 연구를 지속하고 있습니다. 또한 응급실에서의 패혈성 쇼크 환자 대응 및 패킷 치료 이행률 향상에 대한 실증적 분석도 진행 중입니다.

15The objective of this study was to evaluate the prognostic value of C-reactive protein (CRP), procalcitonin (PCT), and their combination for mortality in patients with septic shock. This multicenter, prospective, observational study was conducted between November 2015 and December 2017. Among the blood laboratory test results during CPR of OHCA patients, pH and potassium levels were observed as independent factors associated with survival to hospital discharge, and pH level was considered as an independent factor related to neurological recovery.
The rate and number of adequate CCs were significantly lower with the CO-CPR than with the ST-CPR after 2 and 6 minutes, respectively, and performer fatigue was higher with the CO-CPR than with the ST-CPR during 8 minutes of CPR.
We found that 48 hours of therapeutic hypothermia was more effective in attenuating brain apoptosis than 24 hours of therapeutic hypothermia. We also found that the antiapoptotic effects of therapeutic hypothermia were associated with the suppressions of the cleavage of protein kinase C-δ, the cytosolic release of cytochrome c, and the cleavage of caspase 3.
Among ED patients with suspected infection, sepsis, and septic shock, the newly-developed modified CV/total SOFA score had higher predictive validity and identified more patients at risk of septic mortality.
Septic shock patients admitted to the ED during the daytime exhibited lower sepsis bundle compliance than those admitted at night.
