피지훈 교수의 연구실은 뇌신경계 종양 및 신경발달성 질환의 분자 기전과 임상적 치료 전략을 중심으로 연구를 진행하고 있습니다. 특히 뇌종양 내 줄기세포 특성과 Sox2의 역할, 신경섬유종증후군 등 유전성 질환과 연관된 뇌혈관 이상(모야모야 증후군), 뇌신경종양(예: 내이신경종양, 성상세포종양)의 방사선치료 및 수술적 관리에 중점을 두고 있습니다. 또한 선천성 종양인 꼬리뼈부 종양과 뇌전증 관련 종양의 완전한 제거 전략 개발에도 기여하고 있습니다.

Gamma knife radiosurgery for vestibular schwannomas in NF2 patients provided 5-year tumor control in approximately two-thirds of patients and preserved serviceable hearing in approximately one-third. The rates of other cranial nerve deficits were low, and no secondary malignancy was observed. Radiosurgery should be included in treatment options for NF2 patients.
Sacrococcygeal teratoma (SCT) is an extragonadal germ cell tumor (GCT) that develops in the fetal and neonatal periods. SCT is a type I GCT in which only teratoma and yolk sac tumors arise from extragonadal sites. SCT is the most common type I GCT and is believed to originate through epigenetic reprogramming of early primordial germ cells migrating from the yolk sac to the gonadal ridges. Bifocal GCTs are not restricted to germinomas, as had been previously reported, but do include mixed GCTs. The authors hypothesize that bifocal GCTs may result from the metastatic spread of suprasellar or pineal GCTs. The bifocal presentation of germinomas may be a poor prognostic sign and should alert clinicians to the possibility of a disseminated disease.
Surgical treatment of tumor-related TLE resulted in long-term seizure control in the majority of patients. Maximal tumor removal can be recommended for tumor-related TLE.
Although (18)F-FDG plays a major role in the preoperative work-up of epilepsy surgery patients, it appears from this study that (18)F-FDG does not contribute to the differential diagnosis and that another tracer such as (11)C-methinine is required. (11)C-methinine PET results correlated well with the pathologic spectrum in pediatric lesional epilepsy patients.
The seizure-free rate after surgery in children with FCD was 49% in the 1st year; however, it declined thereafter. The running-down phenomenon could be an important mechanism of seizure alleviation for patients with FCD during long-term follow-up. Because a complete resection of FCD has a strong prognostic implication for seizure control, a better method to define the extent of FCD is required to assist with resection, especially in nonlesional epilepsy.
The use of GKS to treat trigeminal schwannoma resulted in a high rate of tumor control and functional improvement. Cranial neuropathies are bothersome complications of radiosurgery, and tumor expansion in a cavernous sinus after radiosurgery appears to be the proximate cause of the complication. Loss of central enhancement could be used as a warning sign of cranial neuropathies, and for this vigilant patient monitoring is required.
Intracranial germ cell tumors (iGCTs) are a heterogeneous group of tumors with peculiar characteristics clearly distinguished from other brain tumors of neuroepithelial origin.
