연구실 소개
손영제 교수의 연구실은 뇌혈관 질환, 특히 무요모병증(Moyamoya disease)의 수술적 치료 및 장기적인 뇌졸중 예방 전략에 중점을 두고 있습니다. 고령 환자나 의식수준이 낮은 환자에서의 합병증 위험을 분석하고, 혈관 재관류 수술의 임상적 및 영상적 결과를 종합적으로 평가하는 데 기여하고 있습니다. 특히, 병변의 유형에 따른 예후 차이와 위험 인자 분석을 통해 개인화된 치료 전략 수립에 기여하고 있습니다.

15Combined revascularization surgery resulted in satisfactory long-term improvement in clinical, angiographic, and hemodynamic states and prevention of recurrent stroke.
OBJECTIVE The purpose of this study was to evaluate treatment outcomes of future stroke prevention, perioperative complications, and angiographic revascularization in adults with symptomatic moyamoya disease (MMD) according to treatment modalities and surgical techniques. METHODS A systemic literature review was performed based on searches of the PubMed, Embase, and Cochrane Central databases. A fixed-effects model was used in cases of heterogeneity less than 50%. A poor GCS score (≤ 8) and an age of ≥ 65 were found to be related to the occurrence of one of the above-mentioned complications. These results should help neurosurgeons anticipate these complications, to adopt management strategies that reduce the risks of complications, and to improve clinical outcomes.
Prognostic/therapeutic study, level IV.
There was no statistically significant difference in the incidence of stroke in the different clinical groups; clinical status, however, was most severe in patients with hemorrhagic presentation. In patients who experienced stroke during the follow-up period, the stroke type tended to correspond to their initial presentation. Close follow-up is needed in patients with thyroid disease and a family history of MMD.
Prior symptomatic haemorrhage and MR appearance could be related to prospective symptomatic CM haemorrhage in adults. A prospective multicentre observational study is necessary to confirm our results.

During the follow-up period, recurrent hemorrhagic events continued to increase and deteriorated the patients' neurological conditions. The presence of intraventricular hemorrhage was a significant risk factor of recurrent hemorrhage.
