연구실 소개
최영록 교수의 연구실은 장기 이식 분야, 특히 자가 및 생체 기증자에서의 간절단 수술에 중점을 두고 있으며, 특히 복강경을 활용한 기증자 간이식 수술의 안정성과 병변 예후를 분석하고 있습니다. 특히 수술 기술의 최적화와 합병증 예방을 위한解剖학적 요인 분석, 예를 들어 담도 합병증과 관련된 기증자의 해부학적 특성 연구도 진행하고 있습니다. 또한, 바이러스성 간염을 앓는 기증자에서 온 간 이식 graft의 안전성과 유효성에 대한 연구도 함께 수행하고 있습니다.

15This study aims to compare the early outcomes between pure laparoscopic living donor right hepatectomy (PLDRH) and open living donor right hepatectomy (ODRH) after those learning curve. This study reports our experience using deceased donor liver grafts from HBsAg-positive donors. We performed eight cases of liver transplantation (LT) using grafts from deceased HBsAg-positive donors between November 2005 and October 2010. The median age of donors was 48 years (range: 26-64). Solo SILLR is more feasible and safer for liver tumours located in the superficial right liver or in the left one. Therefore, solo surgery can be an option in SILLR.
Surgery yielded greater growth improvement than dietary management. However, after PCS, metabolic abnormalities remained unresolved, and the de novo HCA rate was high. Portacaval shunting can be used to improve growth in GSD type I patients when LT is not possible, but close observation for metabolic abnormalities and HCA is essential.
From our initial experience, RH might be considered as a feasible procedure in the liver resection, even in major hepatectomy. In addition, surgeons with sufficient experience in LH could rapidly adapt for RH. However, we have to make a system for education and monitoring of this innovative surgery for the patients' safety.
