연구실 소개
제형곤 교수의 연구실은 심장외과 및 예방심장학 분야에 중점을 두고 있으며, 심방세동(AF)의 수술적 치료 전략, 특히 비체외순환 수술(ablation)과 하이브리드 치료법의 임상적 유효성과 안정성을 중심으로 연구를 진행하고 있습니다. 또한 대사증후군과 정신건강, 사회경제적 요인 간의 성별 차이에 대한 분석을 통해 심혈관 질환의 예방적 접근법을 모색하고 있습니다. 특히 심내막 및 심막 수술, 미세외과적 수술 기법을 활용한 심장 기형 수술의 미비한 외과적 흉터 문제 해결에도 관심을 기울이고 있습니다.

15There is a growing trend to perform off-bypass surgical ablation for atrial fibrillation (AF) because it is perceived to be safer and more effective than the Cox-Maze procedure with cardiopulmonary bypass (CPB) support. In this systematic review, we compared three minimally invasive stand-alone surgical ablation procedures for AF: the endocardial Cox-Maze procedure, epicardial surgical ablation and a hybrid epicardial surgical and catheter-based endocardial ablation procedure (hybrid procedure).

The present study showed that postoperative long-term survival and event-free survival in patients with active infective endocarditis of the native mitral valve were not statistically significantly different regardless of whether patients underwent MVP or MVR.
For most patients who present with ventricular septal defects (VSDs) in adulthood, the major concern is not the mortality or morbidity associated with repair surgery, but rather cosmetic problems arising from surgical scarring as a result of median sternotomy. The results of our maze procedure during the last decade showed an acceptable success rate and the patients who were restored to sinus rhythm after maze procedures showed better long-term survival rates. For the patients who have independent biological risk factors, more thorough ablation lesion set is recommended for better long-term results.

Improvement in quality of life (QoL) is a primary treatment goal for patients with peripheral arterial disease (PAD). This risk prediction model using a scoring system specific to heart valve surgery was developed from the KHVSR database. The risk prediction model showed that operative mortality could be predicted well in a Korean cohort.
Although the PABD group had higher postoperative hemoglobin levels, there was no clear clinical benefit in the early postoperative period, despite a great deal of effort and additional cost. Additional PABD in the setting of strict policies for blood conservation was ineffective in reducing ABT for young and relatively healthy patients who underwent MICS.
Pump-controlled retrograde trial off is an effective method to safely wean from veno-arterial extracorporeal membrane oxygenation in adult patients. Two cases of catheter entrapment in the mitral valve, which is a rare complication of pulmonary vein isolation, were successfully treated via robot-assisted minimally invasive surgery.
Human brucellosis is a rare zoonosis in East Asia. A case of brucella endocarditis in a 59-year-old farmer who had mild rheumatic mitral stenosis is presented. Excision of the mitral valve with associated vegetation was performed and a mechanical valve was substituted. Antibiotic treatment with doxycycline, rifampicin, and trimethoprim/sulfamethoxazole was continued for 6 months. After 18 months of follow-up, the patient had no symptoms and no signs of relapse.
