연구실 소개
구자원 교수의 연구실은 청각 및 전정 기능의 병리학적 기전을 밝히고, 특히 약물성 난청, 비정상성 내이 해부학적 이상, 전정성 어지러움 등에 기여하는 병태생리학적 메커니즘을 중심으로 연구를 진행하고 있습니다. 특히 항생제 난청의 발생 메커니즘과 내이 약물 도달 경로, 내이 구조의 해부학적 이상이 청각 및 전정 기능에 미치는 영향을 실험적 모델과 임상적 접근을 융합하여 연구하고 있습니다. 또한 노인성 어지러움, BPPV, 내이 박자성 이명 등 신경이비과 질환의 조기 진단 및 치료 전략 개발에도 기여하고 있습니다.

In horizontal canal benign paroxysmal positional vertigo, lying-down nystagmus mostly beats toward the involved ear in the apogeotropic type and directs to the healthy ear in the geotropic type. The direction of lying-down nystagmus may help lateralizing the involved ear in horizontal canal benign paroxysmal positional vertigo.
Vertigo and dizziness are the greatest contributors to the burden of disability in the aged population. Screening for dizziness and vestibular dysfunction, and management of associated factors might be important for improving compromised quality of life due to postural imbalance caused by vestibular problems.
Thinning or dehiscence of the superior semicircular canal may occur on the middle cranial fossa floor or adjacent to the superior petrosal sinus (SPS). ITG failed to control vertigo in 10.5% of cases. ETG may be a reasonable option to facilitate the delivery of gentamicin into the inner ear by direct application of gentamicin over the round window and the oval window. Labyrinthectomy and vestibular neurectomy still have roles in the era of ITG.
Our results demonstrate that patients may develop vestibular symptoms during their clinical course, and all patients with an enlarged vestibular aqueduct should be cautioned regarding the potential development of vestibular pathology. Moreover, the non-negligible incidence of secondary BPPV mandates positional tests when evaluating EVA patients with vertigo.

Comorbid BPPV is a negative prognostic indicator of auditory function in ISSNHL. Concurrent BPPV in ISSNHL suggests combined damage to the utricle and may indicate severe and widespread labyrinthine damage, leading to the poor prognosis.

Among 26 patients with radiologically confirmed EVA, 5 (19.2%) exhibited positional nystagmus compatible with BPPV through Dix-Hallpike and head-roll tests. Hearing loss usually accompanied BPPV attacks, and BPPV was recurrent in three patients. Multiple semicircular canals were frequently involved in each episode, and different canals were also involved in recurrent cases. Canalith repositioning procedures were usually successful without difficulty.
Background and Objectives laser is known to have optimal tissue characteristics for stapes surgery though it has suboptimal optical characteristics. Surgical experience of stapes surgery using laser has not been previously reported in Korea. In this study, authors assessed the functional outcomes of stapes surgery using laser and evaluated its merits and drawbacks. Subjects and Method We retrospectively reviewed the medical records of 25 patients (28 ears) who underwent laser stapedotomy between
37 patients (71%) showed VIN. VIN to ipsilesional side was in 10 and to contralesional side in 27. In patients with CP over 43% (N =23), 2 beated to ipsilesional side, 17 to contralesional side and 4 showed no VIN. In patients with CP of less than 43% (N =29), 8 beated to ipsilesional side, 10 to contralesional side and 11 showed no VIN (p<0.05). 33 patients (63%) showed HSN and 24 patients of them (72%) showed contralesional nystagmus. Transtympanic resurfacing with bone cement, reinforcing the dehiscent JB to reduce focal turbulent flow, is a simple and effective surgical treatment option in patients with PT due to HJBD. In patients with HJBD, the objective measurement of PT by TSR/STA may be of help in selecting appropriate surgical candidates and objective evaluation of the treatment outcome.
