연구실 소개
최정연 교수의 연구실은 노화와 수술 간의 상호작용을 중심으로, 노인 환자의 수술 예후를 정밀하게 예측하고 개선하기 위한 다차원적 평가 체계를 개발하고 있습니다. 특히 종합노인평가(CGA)를 기반으로 한 다차원적 취약도 점수(Hip-MFS)를 활용해 수술 후 합병증, 장기 입원, 사망률 등을 정확히 예측하는 데 초점을 맞추고 있으며, 이는 임상 현장에서의 실질적 적용 가능성을 높이고 있습니다. 또한 ICT 기반의 노인 건강 관리 시스템 개발을 통해 기관 내 거주 노인 환자의 종합적 관리 체계를 구축하고자 합니다.

15High mortality and dependent living after hip fracture pose a significant public health concern. Retrospective study was conducted with 481 hip fracture patients (≥65 years of age) undergoing surgery from March 2009 to May 2014. The Hip-MFS was calculated by Comprehensive Geriatric Assessment (CGA). The primary outcome was the 6-month all-cause mortality rate. Multidimensional Frailty Score based on a preoperative comprehensive geriatric assessment is useful for predicting postoperative complications and prolonged hospital stay, even in low-risk elderly women who are undergoing cancer surgery.
Multidimensional frailty score based on preoperative comprehensive geriatric assessment showed better utility than age, grip strength, gait speed, or ASA classification for predicting post-operative complication and 6 month mortality. MFS also showed incremental predictive ability for post-operative complications with the addition of age and ASA classification. Accordingly, MFS is superior to grip strength or gait speed for predicting complications among older surgical patients.
Both the Hip-MFS and HGS could predict postoperative complications and improve prognostic utility when combined with the ASA classification. The Hip-MFS was a stronger predictor of mortality than HGS after HF surgery. HGS could be a useful pre-screening tool to identify patients at a high risk of postoperative complications and those who may benefit from further CGA.

This study identified the current status and unmet needs of healthcare systems for older adults. We developed an ICT-based system to manage older institutionalized patients. However, the Health-RESPECT service model requires further validation.

Older patients with physical or cognitive frailty had lower BP but higher BP variability. Relationship among frailty, increased BP variability and adverse clinical outcomes should be investigated.
The derived model shall assist clinicians and patients in understanding the individualized risks and benefits of surgery.

Although the patients with a high burden of DAPs were less dependent and had better cognitive and communication functions, they had poorer quality of life. DAP use in LTCH patients should be monitored carefully, and the risk/ benefit relationship for their use should be considered.
