연구실 소개
신경환 교수의 연구실은 방사선 옹호 치료의 정밀화와 치료 결과 최적화를 목표로 하며, 특히 암 치료에서 산소 부족 상태(저산소 상태)의 동역학과 그 영향을 분석하는 데 초점을 맞추고 있습니다. 저산소 상태 마커인 CAIX와 외재적 마커인 피모니다졸을 비교하여 종양 내 산소 수준을 정량화하고, 방사선 치료의 병변 반응 및 합병증 예측 모델을 수립하는 데 기여하고 있습니다. 또한, 딥러닝 기반 자동 병변 추적, 다중센터 임상 연구, 그리고 실시간 방사선 선량 측정 기법을 활용해 치료의 정확성과 안전성을 향상시키는 종합적 접근을 펼치고 있습니다.

15We have used immunohistochemistry to examine the dynamics of tumor hypoxia.. Expression of CAIX is known to be influenced by tumor hypoxia, and this protein has been shown to be an endogenous hypoxia marker in several models. A dose-response relationship between radiation dose and the risk of complications was validated in this multi-center dataset. In this context, we hypothesize that the use of hypofractionated radiotherapy (40 Gy in 15 fractions) may improve breast reconstruction outcomes. Our multi-center prospective observational study (NCT03523078) is underway to further validate this hypothesis.
Patients with pT1-2N0-1 breast cancer who underwent total mastectomy without PMRT could be stratified by nodal stage and risk factors for LRR. PMRT may have of value for node negative patients aged less than 50 years and who are not treated with adjuvant chemotherapy, and for non-luminal type patients with one to three positive nodes.
DL-based auto-contours improved the contour agreement for OARs and CTVs significantly, both qualitatively and quantitatively, suggesting its potential role in minimizing radiation therapy protocol deviation.
This study showed that in vivo dosimetry using TLD during HDR ICR could have a useful role as a predictor of late rectal complications. Dose limitation to the rectum could possibly be achieved by thorough vaginal packing or use of an applicator with proper shielding while maintaining a high dose to the tumor.
Our study demonstrated that a shorter interval from surgery to RT and other established clinical factors were associated with increased lymphedema risk. By combining these factors with two dosimetric parameters, we propose a multivariable logistic regression model for breast cancer-related lymphedema prediction after RT.
A multimodality strategy including NAC, breast surgery, and IMN-covering radiotherapy was effective for patients with overt IMN + breast cancer. Even without an IMN dissection, most patients were IMN failure-free with an IMN-focusing radiotherapy.
