Professor Gieuk Seong's research lab specializes in advanced cardiovascular interventions and critical care technologies, with a strong focus on extracorporeal life support systems such as ECMO and percutaneous cardiopulmonary support (PCPS) in patients with severe cardiac failure or post-cardiac arrest states. Long-segment tracheal stenosis in infants and small children is difficult to manage and can be life-threatening. A retrospective review of 12 patients who underwent surgery for congenital tracheal stenosis between 1996 and 2004 was conducted. The patients’ median age was 3.6 months. 배경: 경피적 심폐 보조장치(PCPS)는 체내의 가스교환과 혈액의 관류를 간접적으로 도와줌으로써, 장기를 회복시키기 위한 다른 치료법의 사용을 용이하게 하고, 여러 다양한 임상 상황에서 출혈의 위험성을 최소화하면서 심각한 심폐부전에 빠진 환자의 생명을 구해준다. 본 저자들은 허혈성 심질환자들에서 발생한 심인성 쇼크 혹은 심정지를 PCPS로 치료한 경험을 보고하고자 한다. 대상 및 방법: 1999년 5월부터 2005년 6월까지 발생한 심정지 혹은 심인성 쇼크 환자들을 대상으로 연구가 진행되었고, 환자들 중 7명에서 원심펌프인 Biopump (Medtronic inc., Minneapolis, MN), 나머지 13명에게는 자가 priming이 되는 헤파린-코팅된 EBS (Terumo, Japan)를 사용하였다. 삽관은 모든 환자에서 대퇴 동맥과 대퇴 정맥을 통해서 이루어졌으며, 동맥용 케눌라는 17 Fr에서 21 Fr까지, 정맥용은 21 Fr에서 28 Fr까지의 DLP (Medtroni

만성폐색전증으로 인한 폐동맥고혈압 환자에서 시행한 폐동맥내막절제술

배경: 만성폐색전증으로 인한 폐동맥고혈압은 국내에 잘 알려져 있지 않으나 매우 나쁜 예후를 보이는 질환이다. 내과적인 치료는 그 효과가 적으며 고식적인 치료방법으로 사용된다. 폐동맥 내막절제술은 수술 술기 및 술 후 관리의 발달에 따라 이 질환의 가장 좋은 근본적인 치료방법으로 알려져 있다. 대상 및 방법: 2001년 1월에서 2005년 12월 사이에 11명의 환자에서 폐동맥 내막절제술을 시행하였다. 모든 환자는 만성적인 호흡곤란과 운동불내성(exercise intolerance)을 나타냈다. 진단은 심장초음파, 폐관류스캔, 컴퓨터 단층촬영을 통해 이루어졌다. 수술 전 한 명의 환자를 제외한 모든 환자에서 하대정맥필터를 삽입하였다. 수술은 극저체온상태에서 전신순환정지를 통해 폐동맥의 최대한 원위부까지 내막절제술을 시행하였다. 중등도 이상의 삼첨판 역류증은 수술 중에 판막성형술로 교정하였다. 결과: 수술사망 및 만기사망은 없었다. 우심실수축기압은 수술 전 91 21 mmHg에서 수술

활동성 심내막염에 대한 조기 수술의 단기 성적

배경 : 활동성 심내막염의 외과적 치료시기에 대해서는 아직 논란의 여지가 있지만 최근에는 조기 수술에 대한 좋은 보고들이 있다. 이에 저자들은 활동성 심내막염에 대한 수술 결과를 수술 전 항생제 사용기간에 따라 분석해 보았다. 대상 및 방법 : 1995년 3월부터 2001년 10월까지 활동성 심내막염으로 삼성서울병원 흉부외과에서 수술 받은 환자 51명에 대해서 후향적으로 분석을 하였다. 남녀 성비는 39:12였고, 평균 연령은 44.5±17.8세(범위 : 13~74세)였다. 감염된 판막은 승모판막이 17례(33.3%), 대동맥판막이 15례(29.4%), 승모판막 및 대동맥 판막이 12례(23.5%), 삼첨판막이 5례(9.8%)였다. 그 중 인공판막 심내막염이 10례(19.6%)였다. 감염 균주는 포도상구균이 19례(37.3%), 연쇄상구균이 17례(33.3%), 장내구균이 3례(5.9%), 진균이 3례(5.9%), 기타균이 5례(9.8%), 균주가 동정되지 않은 경우가 6례(11.8

Right heart failure is a relatively common complication after left ventricular assist device (LVAD) implantation. Severe right heart failure can be managed by temporary right ventricular assist device (RVAD) implantation. However, trans-sternal RVAD insertion requires a subsequent third sternotomy for cannula removal. Herein, we present a case of RVAD insertion via a left anterior mini-thoracotomy after LVAD implantation in a patient with alcohol-induced cardiomyopathy.
