Advance directives (ADs) can support autonomy in making healthcare decisions and minimize unnecessary discomfort during the treatment process at the end of life (EOL). This study aimed to investigate the factors that influence AD documentation among community-dwelling older adults. We used data from the National Survey of Older Koreans which was conducted nationwide in 2020. Data from participants aged 65 years or older were extracted using stratified multistage cluster sampling and the survey w
PURPOSE: Communication with family members is important to end-of-life care for patients with cancer. It is an interactive engagement between terminally-ill cancer patients and their families through which they expand their mutual understanding to cope with losses and find meaning in death. This study aimed to describe the experiences of end-of-life communication between patients with cancer and their family members in South Korea. METHODS: This is a qualitative descriptive study using in-depth
Despite a broad consensus that terminally ill patients should be empowered to choose and deny life-sustaining-treatments (LSTs), South Korea’s path towards the regulation of advance directives was not easy. In this chapter, I sketch the development of social and legal discourse regarding advance directives, from the 1998 Boramae Hospital case to the current Act on Hospice and Palliative Care and Decisions on Life-Sustaining Treatment for Patients at the End of Life (ELDA). I show how ELDA has re
Background: The National Project of Bio-Big Data (NPBBD) is a South Korean bio-big data collection project, expected to include health, genomic, and lifelog data of one million Koreans. The Ethical, Legal, and Social Implications study is a parallel study active since 2020. As part of the study, a public survey was conducted to evaluate public attitudes towards engagement schemes, such as public committees and web portals for communication between the public and researchers. Methods: An online s
Medical ethics has been integrated into the curriculum for medical education in Korea for over 30 years now. There have been many attempts to establish standards for the educational objectives and curriculum, including establishing learning outcomes and publishing medical ethics textbooks in Korean. However, this task is not easy for several reasons: the interdisciplinary nature of the subject, educators’ lack of experience, and the lack of a consensus on the criteria that would be effective edu
This article examines the development of the Republic of Korea's strategy to prevent the spread of COVID-19 with particular focus on ethical issues and the problem of politicization of public communication. Using prominent examples of stakeholders who have acted and expressed themselves in highly contradictory ways on the topic of the pandemic, we provide an analysis of how the public health policy discourse has entered into the realm of politicization and elaborate on the danger that this pheno

The Act on Hospice and Palliative Care and Decisions on Life-Sustaining Treatment for Patients at the End of Life (ELDA) was passed by the Korean National Assembly in 2016 and came into effect in 2018. Although the stated purpose of the act is to uphold patient autonomy and dignity in the context of end-of-life (EOL) care, its utilization remains low, with only some 40% of EOL decisions reflecting patients’ direct opinions. This study aims to pinpoint areas for enhancing ELDA by comparing it wit
In this article casuistical deliberation is explained and examined in regards to whether or not it is suitable for clinical ethics deliberation. Casuistry is the analysis of moral issues using reasoning based on paradigms and analogies. Such case-based reasoning leads to the formation of expert opinions about moral obligations that are general but not universal or invariable. Casuists try to give guidance in cases in which no clear ethical principles can be applied and to categorize cases into p
BACKGROUND: Transplant coordinators face ethical conflicts in various situations, such as deciding who should receive organ donations and how to consider patient costs and such conflicts are expected to be more frequent in Korea, as organ transplant coordinators in the country perform both organ acquisition and transplantation. RESEARCH AIM: This study aims to develop an ethics education program to enhance organ transplant coordinators' ethical competence and address the ethical conflicts faced
As artificial intelligence (AI) technologies advance and become increasingly integrated into medicine and healthcare services, there is a growing consensus that it is necessary to prepare medical students to understand and utilize AI in medical education. Research and discussions are ongoing regarding the AI competencies that healthcare professionals should have. There are diverse opinions on how to integrate the necessary AI competencies for medical graduates into existing curricula. However, w
