15Abstract Background Carbon dioxide (CO 2 ) embolism is a serious, although rare, complication of remote access thyroidectomy using CO 2 insufflation. Recently, we encountered a case of CO 2 embolism during transoral thyroidectomy, and present it here with a review of the literature. Methods and Results A 59‐year‐old female patient with papillary thyroid carcinoma underwent transoral robotic thyroidectomy with CO 2 insufflation. During skin flap elevation, the anterior jugular vein was lacerated.
BACKGROUD: Although propofol and dexmedetomidine have been widely used for monitored anesthesia care, their adverse effects necessitate the search for better methods. Our meta-analysis provided the evidence of improved oxygenation in PCV. However, it is difficult to draw any definitive conclusions due to the fact that the duration of ventilation in the studies reviewed was insufficient to reveal clinically relevant benefits or disadvantages of PCV. Significantly lower peak inspiratory pressure is the advantage of PCV.

PURPOSE: Reconstruction of perineal defects remains a challenge because such defects can be extensive, complex, and three-dimensional. This report presents a retrospective review of our past 5 years of experience in perineal reconstruction, and suggests a simple algorithmic approach according to anatomical divisions with multiple pedicled perforator flaps for extensive perineal defects.
