고혈압의 치료는 크게 생활 양식의 변화와 약물요법 으로 나눌 수 있으며, 약물 치료를 할 때는 고혈압의 병태 생리와 혈역동학적 변화, 관상동맥 질환의 위험 인자, 동반하고 있는 타 질환, 부작용, 환자의 순응도, 삶의 질 등 여러 가지 요소들을 고려하여 적절한 강압 제를 선택하는 것이 중요하다. 고혈압을 체계적으로 치료하기 위해 세계 각국에서의 여러가지 치료지침이 보고되어 왔다. 즉 1993년의 세계 보건 기구 및 국제 고혈압 학회(WHO/ISH)의 치료 지침, 미국의 제 5 차 합동 조사 연구 보고(JNC V), 카나다 고혈압 학 회, 영국 고혈압 학회의 보고와 1992년의 뉴질랜드 보고 1994년의 호주의 보고 등이 발표 되었다. 그러 나 이들 치료 지침들 사이에는 공통점이 있는 반면 여 러가지 문제점에 대한 이견과 결정 짓지 못하고 있는 사항도 있는게 사실이다. 1993년과 1994년에 보고된 치료 지침에서의 공통 된 점을 보면 심혈관계 위험인자의 중요성, 노인성 고 혈압 및

배경 및 목적： 최근 좌심실 질량의 결정인자에 대한 연구 영역이 혈역학적 인자에 국한되지 않고 비혈역학적 인자로 확대되면서 좌심실 질량의 적정성의 평가에 대한 연구가 활발해지고있다.1)2) 좌심실 질량의 적절성을 평가하기 위해서는 성장과 더불어 적절한 좌심실 증가에 관여하는 신장, 성별, 심박출일의 영향이 성인에서 어떻게 좌심실 질량과 관계되는지의 연구가 필요하다. 이에 본 연구는 한국인에서 좌심실 질량을 결정하는데 있어서 이들 인자의 관련성을 규명하기 위하여 시행되었다. 방 법：한양대학병원 심초음파실과 양평지역사회의학 연구 집단의 심초음파 데이터 베이스에서 정상체중, 정상 혈압인 성인 368명(남자 184명, 여자 184명)을 대상으로 하였다. 좌심실 질량은 1.04×(IVSd+LVDd+PWTd)3-LVDs3)×0.8-0.6 으로 계산하였고 일회심박출량은 Teichholz방정식에 의해 계산하였으며 일회심박출일은 수축기혈압×일회심박출량×0.00144로 계산하였다. 결 과：일회심박출일
