연구실 소개
임도훈 교수의 연구실은 위암, 간세포질환, 췌장암 등 소화기 종양의 치료 전략 개발에 초점을 맞추고 있으며, 특히 신재생의학 기반의 보조 치료법과 고정도 방사선 치료(SBRT, 3D-CRT)의 임상적 적용을 중심으로 연구를 진행하고 있습니다. 특히, 수술 이전 또는 이후 적용되는 보조 치료 전략, 방사선 치료의 최적 용량 및 기법 선정, 그리고 인공지능 기반 생존 예측 모델 개발을 통해 개인화된 종양 치료의 실현 가능성을 모색하고 있습니다. 최근에는 방사선 치료의 병용 요법 및 병변 특성에 맞는 정밀 방사선 치료 기법 개발에도 주력하고 있습니다.

BACKGROUND: Portal vein thrombosis (PVT) is a common complication in patients with advanced-stage hepatocellular carcinoma (HCC). The authors evaluated the impact of radiotherapy (RT) for PVT of HCC and analyzed the dose-response relation between RT and PVT. Early 3D-CRT for HCC unresponsive to 1 or 2 cycles of TACE resulted in a 62.8% tumor response rate and relatively high complete response rates (20.9%) with acceptable toxicity. This study shows that the application of 3D-CRT could be considered for patients with incomplete TACE.
Lymph node metastasis was a poor prognostic factor and adjuvant RT may be useful in patients with microscopic residual tumor. However, because there were high loco-regional recurrences, additional study is needed to determine more effective RT such as increased RT dose or use of radiosensitizers.

High-dose 3D-CRT for small HCC <5 cm showed favorable local control and overall survival without serious complications. RT might be an effective local modality in patients with small HCC not eligible for other local modalities.
