연구실 소개
이은지 교수 연구실은 망막 혈관망의 미세혈관 변화와 시신경축삭의 퇴행 간의 연관성을 중심으로, 고해상도 산성광학간섭단층촬영(angiography, OCTA)을 활용한 논문 연구를 진행하고 있습니다. 주로 원위부 망막신경섬유층의 퇴행과 관련된 미세혈관 소실(MvD), 특히 시신경두부 주변의 깊은층 혈관망 변화를 분석하여, 당뇨병성 망막병변이나 녹내화의 조기 진단 및 진행 추적에 기여하고자 합니다. 특히, 시신경두부 주변의 망막 및 망막하 조직의 미세해부학적 변화와 혈류 이상 간의 상관관계를 규명하는 데 초점을 맞추고 있습니다.

15Decreased parapapillary microvasculature of the retina determined by OCTA was found at the location of RNFL defect in POAG patients. This finding suggests that the decreased retinal microvasculature is likely secondary loss or closure of capillaries at the area of glaucomatous RNFL atrophy.
MvD was identified in the parapapillary deep layer exclusively in POAG eyes at the location of glaucomatous damage using optical coherence tomography angiography. The finding suggests that peripapillary deep-layer circulation is directly related to the glaucomatous optic neuropathy.
Microvasculature dropout enlargement was associated with progressive RNFL thinning in POAG.
The peripheral LC exhibited a recent alteration in eyes with DH. The alteration was correlated spatially with the location of the DH. These findings suggest that DH may result from microvascular damage incurred from alteration of the LC near its insertion.
Within the limits of retest variability, Stratus OCT detects progressive RNFL atrophy with high sensitivity and moderate specificity in cases showing localized progressive loss of retinal nerve fibers in red-free photographs. The specificity can be improved by use of multiple measurements. Stratus OCT is a potentially useful technique for detection of glaucoma progression.
Purpose: To characterize the microstructure underlying the parapapillary deep-layer microvasculature dropout (MvD) identified by optical coherence tomography (OCT) angiography (OCTA) in eyes with primary open-angle glaucoma (POAG). The present study investigated 22 cases of peripapillary retinoschisis in glaucomatous eyes. The retinoschisis was attached to the optic nerve and topographically correlated with RNFL defect. It often resolved spontaneously without causing severe visual disturbance. Care should be taken not to overestimate the RNFL thickness in eyes with retinoschisis, and also not to misinterpret the resolution of retinoschisis as a rapid glaucomatous RNFL deterioration.
The rate of OCT RNFL thinning was significantly greater in patients with progressive localized RNFL defects than in those with stable localized defects. The data suggest that trend-based analysis of OCT RNFL thickness may be useful in glaucoma progression analysis and may complement other diagnostic tests.
