15BACKGROUND: The preoperative localization of thoracodorsal artery perforators has been a challenge because of their topographic variation. METHODS: From November of 2005 to January of 2007, 25 patients underwent free thoracodorsal artery perforator flap transfer for reconstruction of the lower extremities (22 flaps) and the head and neck (three flaps). All of the patients underwent multidetector-row computed tomographic angiography for preoperative perforator mapping. The computed tomographic fi
The extended latissimus dorsi flap has been widely used for breast reconstruction. However, seroma at the donor site is a common complication and makes it difficult for reconstructive surgeons to choose it as a primary option. We analyzed the association between seroma and reconstructions with extended latissimus dorsi flaps. A series of 120 consecutive cases were included in this study. The average body mass index (BMI) was 22.1 kg/m, and the mean ratio of the flap weight to the extirpated brea
BACKGROUND: The anatomical topography of the perforators that pierce the latissimus dorsi muscle fascia determines the location and direction of the skin paddle and thus the ultimate design of the thoracodorsal artery perforator flap. The full scope of the thoracodorsal artery perforator topography has not been provided in detail, and the usefulness of perforators from the transverse branch has not been elucidated. METHODS: While harvesting the thoracodorsal artery perforator flap, every reliabl

PURPOSE: Perfusion zones of the deep inferior epigastric perforator (DIEP) flap aid surgeons in estimating approximate tissue territory with reliable perfusion. Since the Hartrampf perfusion zone was developed, it has been modified continuously; however controversies remain regarding whether the recent perfusion models can satisfy current anatomical and clinical evidence. We conducted a comprehensive review of previous studies to synthesize relevant knowledge and critically reappraise current pe
BACKGROUND: Severe postburn neck contractures are devastating functional and cosmetic deformities. The ideal material for coverage of defects created by neck contracture release is thin, supple, large, well-vascularized, healthy tissue. The thoracodorsal artery perforator flap, a workhorse flap for reconstruction, allows a large dimension and free thickness control with low donor-site morbidity. The value of a free thin thoracodorsal artery perforator flap for anterior neck burn deformities has
BACKGROUND: Despite the decrease in donor-site morbidity with the advent of deep inferior epigastric artery perforator (DIEP) flap breast reconstruction, abdominal complications still occur. There have been few studies on donor morbidity considering the ethnic differences in the Asian population, as represented by a lower BMI with less redundant tissue and a tendency for poor scarring. In the present study, the authors investigated the incidence of abdominal complications and their risk factors
Full exposure of the medial orbital wall for fracture repair poses difficulty with traditional approaches except coronal incision, especially in cases of wide fracture. The endoscopic-assisted approach with limited incision has been introduced. The authors used the endoscopically assisted transconjunctival approach in 21 cases: 15 isolated medial orbital wall fractures and 6 cases of concomitant floor fractures. Through the medial transconjunctival slit incision, repair of the fracture using cal
PURPOSE: Despite extensive previous investigations regarding the efficacy of lymphaticovenular anastomosis (LVA) for the treatment of lymphedema, few studies have reported the long-term clinical course of individual patients undergoing LVA. The present study presented our experience of LVA and to report serial postoperative courses. METHODS: A retrospective chart review was performed for 17 patients (2 male and 15 female) with lymphedema refractory to conservative treatment who underwent LVA and
BACKGROUND AND OBJECTIVES: This study aimed to evaluate treatment outcomes of acral melanoma (AM) based on the excision margin. METHODS: A retrospective cohort study was conducted for patients with primary AM, analyzing recurrence rates, local and in-transit recurrence-free survival (LITRFS), disease-free survival (DFS), and melanoma-specific survival (MSS). RESULTS: Data from 129 patients of AM were analyzed. In 53 patients with thin AM (thickness ≤1 mm), neither recurrence nor mortality occurr
BACKGROUND: This study investigated which zonal tissue would be more secure from the risk of fat necrosis between Holm zones II and III and examined the risk factors of fat necrosis in a clinical series of medial row perforator-based deep inferior epigastric artery perforator (DIEP) flaps. PATIENTS AND METHODS: A retrospective chart review was performed for patients undergoing unilateral breast reconstructions with medial row perforator DIEP flaps. Data regarding patients, operation-related char
