Residual kidney function (RKF) contributes to improved survival in hemodialysis (HD) patients. However, it is not clear whether RKF allows a comparable survival rate in patients undergoing twice-weekly HD compared with thrice-weekly HD.We enrolled 685 patients from a prospective multicenter observational cohort. RKF and HD adequacy was monitored regularly over 3-year follow-up. Patients with RKF were divided into groups undergoing twice-weekly HD (n = 113) or thrice-weekly HD (n = 137). Patients

Our study demonstrates that paricalcitol pretreatment prevents renal IRI via the inhibition of renal inflammation, and the up-regulation of COX-2 and PGE2 is one of the protective mechanisms of paricalcitol in renal IRI.

SJR Q3Seminars in Dialysis

Numerous designs for tunneled hemodialysis catheter have been developed in an effort to improve catheter function and survival. In this prospective randomized controlled study, 97 patients were randomized into the palindrome catheter group (PC, n = 47) and step-tip catheter group (SC, n = 50). Demographic characteristics were not different between the two groups. The effective blood flow rates at different pump speeds were comparable between the two groups. The recirculation was low within accep

Tenofovir‐associated Fanconi syndrome and nephrotic syndrome in a patient with chronic hepatitis B monoinfection
Hyeon Seok Hwang, Cheol Whee Park, Myeong Jun Song
SJR Q1Hepatology

Tenofovir disoproxil fumarate (TDF) is widely used as an effective first-line therapy for chronic hepatitis B (CHB) infection. While TDF demonstrates successful viral suppression, it has been linked to the development of renal proximal tubular (PT) dysfunction, leading to Fanconi syndrome. However, Fanconi syndrome has been rarely reported in CHB-monoinfected patients, and there were no reports of TDF-associated nephrotic syndrome. Here, we report a case of combined Fanconi and nephrotic syndrom

SJR Q1Journal of Clinical MedicineOA

Proprotein convertase subtilisin/kexin type 9 (PCSK9) is a promising new target for the prevention of cardiovascular (CV) events. However, the clinical significance of circulating PCSK9 is unclear in hemodialysis (HD) patients. A total of 353 HD patients were prospectively enrolled from June 2016 to August 2019 in a K-cohort. Plasma PCSK9 level was measured at the time of study enrollment. The primary endpoint was defined as a composite of CV event and death. Plasma PCSK9 level was positively co

SJR Q2BMC NephrologyOA

BACKGROUND: Rifampin is one of the most important drugs in first-line therapies for tuberculosis. The renal toxicity of rifampin has been reported sporadically and acute tubulointerstitial nephritis (ATIN) is a frequent histological finding. We describe for the first time a case of ATIN and Fanconi syndrome presenting as hypokalemic paralysis, associated with the use of rifampin. CASE PRESENTATION: A 42-year-old man was admitted with sudden-onset lower extremity paralysis and mild renal insuffic

SJR Q1Nephrology

AIM: Proteinuria is a primary factor requiring treatment in immunoglobulin (Ig)A nephropathy. The purpose of this study was to assess the relevance of treatment response and relapse of proteinuria with renal function decline. METHODS: One hundred and twenty-five biopsy-proven primary IgA nephropathy patients who had more than 1.0 g/day proteinuria at the first assessment were studied. All patients underwent anti-proteinuric treatment, and the association of the rate of renal function decline wit

Vascular calcification and left ventricular hypertrophy in hemodialysis patients: interrelationship and clinical impacts

SJR Q2International Journal of Medical SciencesOA

We examined the relationship and combined effect of vascular calcification (VC) and left ventricular hypertrophy (LVH) on deaths and cardiovascular events (CVEs) in hemodialysis (HD) patients. Methods: Maintenance HD patients (n=341) were included. Echocardiography data and plain chest radiographs were used to assess LVH and aortic arch VC. Results: VC was found in 100 patients (29.3%). LVH was more prevalent in patients with VC compared with those without VC (70% vs. 50.2%, P=0.001). VC was ind

Paricalcitol Pretreatment Attenuates Renal Ischemia‐Reperfusion Injury via Prostaglandin E2 Receptor EP4 Pathway

The protective mechanism of paricalcitol remains unclear in renal ischemia‐reperfusion (IR) injury. We investigated the renoprotective effects of paricalcitol in IR injury through the prostaglandin E 2 (PGE 2 ) receptor EP4. Paricalcitol was injected into IR‐exposed HK‐2 cells and mice subjected to bilateral kidney ischemia for 23 min and reperfusion for 24 hr. Paricalcitol prevented IR‐induced cell death and EP4 antagonist cotreatment offset these protective effects. Paricalcitol increased phos

BACKGROUND/AIMS: Atrial fibrillation (AF) often coexists with acute myocardial infarction (AMI), and chronic kidney disease (CKD) is a major risk for AMI. However, the combined impact of CKD and AF on the mortality and morbidity in AMI population has not been determined. METHODS: Between January 2004 and December 2009, a total of 4,738 AMI patients were enrolled prospectively. Patients were divided into four groups according to the combined status of CKD and AF. The primary endpoint was a combin

SJR Q1Nephrology Dialysis Transplantation

BACKGROUND: Metabolic syndrome (MetS) is prevalent in patients with end-stage kidney disease, and kidney transplantation is expected to modify the metabolic status. However, whether changes in metabolic status at the time of transplantation affect recipient outcomes remains unclear. METHODS: We analyzed 4187 recipients registered in a nationwide prospective cohort from 2014 to 2020. MetS was defined as the presence of three or more components of the metabolic syndrome. Patients were classified b
