연구실 소개
신현철 교수의 연구실은 경추 및 요추의 척추외과적 수술과 보조 기법에 중점을 두고 있으며, 특히 인공디스크 치환술(ADR)과 전경추간판제거후유합술(ACDF)의 임상적 및 영상적 결과 분석에 힘쓰고 있습니다. 신경학적 합병증인 경막외부 혈종, 골화성 연골형성증(HO), 척추 정렬 이상 등 수술 후 합병증의 발생 원인과 예후 요인을 체계적으로 분석하며, 수술 전후 척추의 정렬과 생체역학적 변화에 대한 정밀한 평가를 수행하고 있습니다. 특히, 수술 후 신경학적 악화를 조기에 진단하고 예방하기 위한 진단적 접근법 개발에도 기여하고 있습니다.

Occurrence of HO is an inevitable postoperative complication after cervical ADR. The occurrence rate of HO was higher than our expectation. Moreover, definite differences in occurrence rate according to the prosthesis type were identified by this study.

Arthroplasty using the Bryan disc provided a favorable clinical and radiologic outcome thus far; however, we should not underestimate emergent adverse outcomes. To prevent postoperative sagittal imbalance after cervical arthroplasty, intentional modification in disc insertion angle and depth would be helpful. Unexpected compensatory biomechanical loads should be elucidated in future studies.

Although bony fusion had been completed, the effects of lumbar arthrodesis on adjacent segments could vary according to the surgical technique used for arthrodesis. Semirigid fixation combined with arthrodesis deserves careful consideration and further detailed study because it may cause less stress on adjacent segments than rigid fixation while maintaining the benefits of the latter procedure.
This study sought to determine the outcomes of posterior lumbar interbody fusion (PLIF), via a unilateral approach, in selected patients who presented with unilateral leg pain and segmental instability of the lumbar spine.
