Gait Patterns of Chronic Ambulatory Hemiplegic Elderly Compared with Normal Age-Matched Elderly
김준호, 오세인, 조형필, 김희상, 전진만, 이원재, 신준호, 안재용, 김태곤, 한정수, 장혜연, 한창수

This study compares the gait of chronic ambulatory hemiplegic elderly (HE) with those of non-hemiplegic elderly (NHE) usingVicon® 512 motion analysis system to track movement trajectories. Subjects walked with self-selected gait speeds while temporal, spatial, and kinematic variables were analyzed. HE showed significantly decreased walking speed, cadence, stride length, and steplength, and increased stride time, step time, and double limb support periods in comparison with NHE. Double limb suppor

Journal of Korean Medical Science

To investigate the contribution of the interleukin-6 receptor (IL-6R) gene single nucleotide polymorphisms (SNPs) to the neurological status of Korean patients with ischemic stroke (IS), two SNPs of the IL-6R gene (rs4845617, 5 UTR; rs2228144, Ala31Ala) were selected. IS patients were classified into clinical phenotypes according to two well-defined scores: the National Institutes of Health Stroke Survey (NIHSS) and the Modified Barthel Index scores. There were 121 IS patients and 291 control su

The objective of study was to compare biomechanical parameters between normal and arch building gait in healthy subjects. A total of 40 feet from 20 healthy adults were evaluated in this study. The participants were asked to walk on a treadmill comfortably at 2 km/h for 30 seconds. Then they were asked to walk after making arch building through raising arches with their feet by pulling the big toe toward the heel. Gait parameters such as geometry, center of pressure, maximum force, and maximum p

We studied newly developed assistive robot movement (ARM, HEXAR-KR40P) equipment, and compared with conventionalcontinuous passive movement (CPM) equipment, focusing on range of motion (ROM) and strengthening of knee joint muscles. 110osteoarthritis (OA) patients were recruited from five different clinics. At first, eighty patients were enrolled to evaluate the subjectivesatisfaction of ARM. Secondly, thirty patients were enrolled to find the effectiveness of ARM including ROM and strength compa

Journal of Korean Medical Science

Phantom limbs are usually observed after amputation of extremities. In patients after a stroke, a similar but rarely occurring phenomenon consisting of the patient experiencing the presence of an additional limb has been described. This phenomenon, generally called supernumerary phantom limb (SPL), may be caused by lesions in the right or left cerebral hemisphere, but has been predominantly reported in patients who have had a right hemispheric stroke. We report two cases of atypical SPL and phan

Monomelic Amyotrophy (Hirayama Disease) With Upper Motor Neuron Signs: A Case Report
유승돈, 이성용, 김희상, 윤동환, 김동환, 전진만, 이승아, 한유진

Monomelic amyotrophy (MMA), also known as Hirayama disease, is a sporadic juvenile muscular atrophy in the distal upper extremities. This disorder rarely involves proximal upper extremities and presents minimal sensory symptoms with no upper motor neuron (UMN) signs. It is caused by anterior displacement of the posterior dural sac and compression of the cervical cord during neck flexion. An 18-year-old boy visited our clinic with a 5-year history of left upper extremity pain and slowly progressi

Thoracic Outlet Syndrome Caused by Schwannoma of Brachial Plexus
윤동환, 김희상, 전진만, 이종언, 정필교

Schwannomas are benign, usually slow-growing tumors that originate from Schwann cells surrounding peripheral, cranial, or autonomic nerves. The most common form of these tumors is acoustic neuroma. Schwannomas of the brachial plexus are quite rare, and symptomatic schwannomas of the brachial plexus are even rarer. A 47-year-old woman presented with a 1-year history of dysesthesia, neuropathic pain, and mild weakness of the right upper limb. Results of physical examination and electrodiagnostic s

Objective To investigate whether four single nucleotide polymorphisms (SNPs) rs2293054 [Ile734Ile], rs1047735 [His902His], rs2293044 [Val1353Val], rs2682826 (3’UTR) of nitric oxide synthase 1 (NOS1) are associated with the development and clinical phenotypes of ischemic stroke. Methods We enrolled 120 ischemic stroke patients and 314 control subjects. Ischemic stroke patients were divided into subgroups according to the scores of the National Institutes of Health Stroke Survey (NIHSS, <6 and ≥6)

Objective To evaluate the relationships between tongue pressure and different aspects of the oral-phase swallowing function. Methods We included 96 stroke patients with dysphagia, ranging in age from 40 to 88 years (mean, 63.7 years). Measurements of tongue pressure were obtained with the Iowa Oral Performance Instrument, a device with established normative data. Three trials of maximum performance were performed for lip closure pressure (LP), anterior hard palate-to-tongue pressure (AP), and pos

Objective: To assess the correlation between surface electromyography (SEMG) and conventional EMG in patients with facial nerve palsy. Additionally, compare the discomfort and the time required by the patients in two methods. Method: 36 patients with facial palsy were given nerve conduction studies (NCS) via conventional EMG. Then, the peak root mean square (RMS) values were obtained from the SEMG. We also recorded visual analogue scale (VAS), House- Brackmann scale, and the time required for th

Objective To evaluate the association between balance function and asymmetry of knee extension strength in an elderly Korean population. Methods The strength of the knee extensors in each leg was measured in 306 community-dwelling elderly subjects (age, 76.70±4.85 years) and 25 young healthy subjects (age, 34.23±8.93 years). Based on the difference in strength of both legs, the elderly subjects were divided into symmetric (n=128) and asymmetric (n=178) strength groups using an asymmetry cutoff 2

Sarcopenia is an age-related condition which is characterized by a decrease in the mass and function of skeletal muscle and can affect the muscles of the trunk and extremities and the muscles to be used in swallowing. Early diagnosis and appropriate intervention of sarcopenic dysphagia is very crucial for older adults. Sarcopenic dysphagia can be diagnosed by measuring the function, strength, and mass of the affected muscle. In addition, dietary function assessments, functional eating scales and

Hemiplegic stroke patients may have abnormal awareness or perception of the affected limb (s). For example, patients may experience their limb as not belonging to them (asomatognosia) or attribute their own body parts to other persons (somatoparaphrenia). Disturbed sensation of limb ownership (asomatognosia, somatoparaphrenia) for the hemiplegic limb has been reported in patients with right insula lesion. We report a case of a 70-year-old right handed female who had somatoparaphrenia and neglect

Dystonia is a movement disorder characterized by involuntary muscle contractions. Patients with dystonia may experience uncontrollable twisting, repetitive movements, or abnormal posture. A 55-year-old man presented with an involuntary left forearm supination, which he had experienced for five years. There was no history of antecedent trauma to the wrist or elbow. Although conventional therapeutic modalities had been performed, the symptoms persisted. When he visited our hospital, electromyograp
