연구실 소개
윤지훈 교수의 연구실은 미생생물의 정확한 종별 동정과 항생제 내성 메커니즘을 중심으로 한 감염병 분야의 분자생물학적 연구를 수행하고 있습니다. 특히, 아에로모나스 속 세균의 정밀 동정을 위한 MALDI-TOF MS 및 유전자 분석 기법을 비교·평가하며 임상적 진단의 정밀도를 높이고자 합니다. 또한 약물 대사 효소인 CYP2C9의 유전자 변이가 약물 반응에 미치는 영향을 분석하여 개인 맞춤형 의료의 기초를 마련하고자 합니다. 최근에는 만성 신장질환 환자의 혈액 투석 시작 시점이 생존에 미치는 영향과 체내 호르몬 비율이 심혈관계 예후에 미치는 영향에 대해서도 임상 epidemiology적 접근을 시도하고 있습니다.

Human cytochrome P450 2C9 is a highly polymorphic enzyme that is required for drug and xenobiotic metabolism. Here, we studied eleven P450 2C9 genetic variants—including three novel variants F69S, L310V, and Q324X—that were clinically identified in Korean patients. 저자들은 수술 후 발열과 함께 복막염이 의심되는 환자에서 시행한 복막액 배양에서 세 건의 드문 Bacteroides 균종을 분리하였다. 세 균주들은 Rapid ID 32A (bioMérieux, France) 검사로 각각 B. fragilis/B. stercoris와 B. ovatus로 동정되었으나, MALDI-TOF MS와 16S rRNA 염기서열 분석을 통해 B. nordii 1예와 B. salyersiae 2예로 확인되었다. 세 균주는 항혐기성 약제인 piperacillin/tazobactam, imipenem, meropenem, chloramphenicol 및 metronidazole에 대해서 모두 감수성이었으며, 환자들은 piperacillin/tazobactam 치료 후 호전되었다. 본 증례를 통해 드문 B. fragilis군균종 동정시에 MALDI-TOF MS가 16S rRNA 염기서열 분석 결과와 일치도가 높고, 간편하고 신속하게 결과를 확인할 수 있다는 점에서 유용
