연구실 소개

김남규 교수의 연구실은 마취 및 통증 관리 분야에서 첨단 내시경 수술과 로봇 수술에서의 안전성 향상에 중점을 두고 있습니다. 특히 CO₂ 인플레이션을 동반한 내시경 갑상선 수술에서 발생할 수 있는 혈관 색소 등 심각한 합병증의 예방 및 대처 전략을 연구하고 있으며, 마취 유도제의 조합 효과 및 통증 관리 전략의 최적화에도 기여하고 있습니다. 또한, 복합성 외과 재건 수술, 특히 골반 부위의 다차원적 결손 재건에 있어 다수의 피드러 박출편역상(피드러) 플랩을 활용한 알고리즘 기반의 재건 전략을 개발하여 임상적 실용성을 높이고 있습니다.

Carbon dioxide embolism during transoral robotic thyroidectomy: A case report

Abstract Background Carbon dioxide (CO 2 ) embolism is a serious, although rare, complication of remote access thyroidectomy using CO 2 insufflation. Recently, we encountered a case of CO 2 embolism during transoral thyroidectomy, and present it here with a review of the literature. Methods and Results A 59‐year‐old female patient with papillary thyroid carcinoma underwent transoral robotic thyroidectomy with CO 2 insufflation. During skin flap elevation, the anterior jugular vein was lacerated.

However, it is difficult to draw any definitive conclusions due to the fact that the duration of ventilation in the studies reviewed was insufficient to reveal clinically relevant benefits or disadvantages of PCV. PURPOSE: Reconstruction of perineal defects remains a challenge because such defects can be extensive, complex, and three-dimensional. This report presents a retrospective review of our past 5 years of experience in perineal reconstruction, and suggests a simple algorithmic approach according to anatomical divisions with multiple pedicled perforator flaps for extensive perineal defects.
