15Purpose: We wished to compare the clinical effectiveness of cystatin C (CyC) and serum creatinine (sCr) to assess renal function in renal transplantation patients. Methods: We compared the clinical effectiveness of CyC with that of the sCr to estimate 24-hour urine creatinine clearance (CrCl) in 72 adult recipients who underwent renal transplantation from January 2001 to December 2008. We analyzed the data in terms of accuracy, bias, precision and sensitivity as a function of length of time post
Purpose: During the high-stake examinations such as OSCE(Objective structured clinical examination) or CPX(clinical performance exam), test security is generally accepted as a major concern for test validity. This study was conducted to investigate the effect on examinee's scores of repeated, serial administrations of essentially the same standardized patient(SP)-based performance exam. Methods: A performance-based examination using eight SP cases was administered to 123 senior medical student
Transplant tourism (TT) has developed into a global concern for international organizations, transplant communities, researchers, and the press. Increasing the knowledge of organ trafficking and TT is essential to raise awareness and prepare responses that will prevent the occurrence of illicit organ transplantation through TT. This review article describes the current status and legal framework of international organ trafficking and TT. Collection and analysis of data concerning TT from nationa
Splenosis is defined as the autotransplantation of splenic tissue after a traumatic splenic injury or a splenectomy. It has often been mistaken as an endometriosis, an intraperitoneal carcinoma, a hemangioma and an accessory spleen. It is clinically significant in that it requires no treatment if it is asymptomatic. Here we report a case of 34 year-old woman who had a pelvic splenosis with cyclic abdominal pain for several years, which was initially misdiagnosed as ovarian dermoid cyst. At the t
An obturator hernia is a rare condition, occurring most frequently in elderly and debilitated women. The diagnosis is difficult, and delayed treatment is associated with serious complications. In most cases, it produces a small bowel obstruction with high morbidity and mortality. We experienced an unusual case of a left recurrent obturator hernia. The patient was a 76-year-old woman with atrial fibrillation, presented with intermittent generalized abdominal pain and distension. The patient under
A 27-year-old woman with a 4-month history of right upper quadrant dull pain and normal blood pressure was admitted in May 2001. Abdominal ultrasonography and abdominal computed tomography revealed a huge retroperitoneal mass with hepatic nodules. Histologic diagnosis was paraganglioma based on gun biopsy. Hormonal study for pheochromocytoma was negative. I-metaiodobenzylguanidine (MIBG) scintigraphy and angiography were performed for operation. The tumor was removed en bloc with part of the pan
Adventitial cystic disease of popliteal artery is a rare cause of lower extremity claudication in the middle aged. We diagnosed an adventitial cystic disease using duplex sonography and spiral CT angiography and operated on a 58 year old man with progressive lower extremity claudication for 4 months. An operation was performed by removing all cystic lesions through the posterior approach of popliteal fossa and interposition graft 6mm PTFE. Preoperative claudication symptom was disappeared and po
Spontaneous pseudoaneurysm of the superior mesenteric artery (SMA) is rare. However, it may have severe life-threatening complications such as ruptures or thrombosis. Our patient has developed a pseudoaneurysm, which was being misdiagnosed as superior mesenteric vein dissection by the computed tomography angiography. The pseudoaneurysm is revealed from branches of the SMA with no specific causes and is being treated by endovascular embolization after emergency exploratory laparotomy.
Malignant fibrous histiocytoma (MFH) is a common sarcoma of the soft tissue, usually occurring in the extremities and less commonly in the retroperitoneal space, the abdominal cavity, or other site such as the skin or the head and neck resion. However, MFH of the liver is quite a rare neoplasm. This tumor has five distinct histological subtypes: storiform pleomorphic, myxoid, giant cells, inflammatory and angiomatoid. Recently, we experienced a case of primary MFH in a surgically removed liver.
Purpose: Multiple primary cancers are not a rare disease. The clinicopathologic characteristics of a hepatocellular carcinoma with extrahepatic malignancies, have not been identified. To address these points, the clinical data of patients with a hepatocellular carcinoma, with extrahepatic malignancies, treated with surgical resections at our hospital, were retrospectively reviewed. Methods: We reviewed the clinical records of ten hepatocellular carcinoma patients, with extrahepatic malignancies,
A primary hepatic sarcoma is a rare tumor, that most frequently arises from hepatic connective tissue or vascular channels, and is usually located in the intrahepatic area. Pedunculated, or bulging, lesions have also been reported. We encountered a pedunculated primary hepatic leiomyosarcoma occurring in a 61-year-old woman. A giant exophytic hepatic mass measuring 15x10cm in size, was located in the left lateral segment of the liver, which was compressing the stomach. A left lateral segmentecto
Adult polycystic liver disease (APLD) is an inherited, benign rare condition, often associated with polycystic kidney disease. Liver failure is unusual, but some patients may require therapy. Surgery appears to be more effective in relieving the symptoms of APLD for an extended period than nonsurgical therapies. We report on the successful surgical treatment of a case of APLD located in the left lobe of the liver. (J Korean Surg Soc 2002;63:171-174)
Purpose: Hyperhomocysteinemia is accepted as an independent risk factor for peripheral arterial disease (PAD). The purpose of this study is to evaluate the correlation between the preoperative plasma homocysteine concentration and restenosis after therapeutic revascularization. Methods: We retrospectively analyzed the clinical records of 58 consecutive patients (they were confined to Trans Atlantic Inter-Society Consensus [TASC] type C & D) among 103 patients who were diagnosed as having infrain
Background: Infections are a major cause of morbidity, graft failure, and mortality in solid organ transplant recipients. Preventive measures have greatly reduced the burden of posttransplant infections. However, little is known about the practice patterns of infection prevention in South Korea. Methods: A questionnaire-based cross-sectional survey was conducted. The questionnaire was developed by a multidisciplinary discussion. From the Korean Network for Organ Sharing data, a list of hospitals
