연구실 소개
금웅섭 교수의 연구실은 종양의 혈관생성 및 혈류 특성에 초점을 맞춘 정량적 MRI 기반 생체지표 개발과 함께, 병변의 혈관 밀도와 약물 침투도를 분석하여 암 치료 반응을 예측하는 데 주력하고 있습니다. 특히 대장암 등 실질성 종양 모델을 활용한 DCE-MRI 분석과 함께, 방사선 치료의 효능 및 예후를 평가하는 임상 연구를 병행하고 있습니다. 최근에는 국소 진행성 직장암 환자에서 수술을 거부하는 환자 대상으로 보조 치료 후 병리학적 완전 반응의 임상적 의미를 규명하는 데에도 기여하고 있습니다.

목 적: 방광압박도구(bladder compression device) 추가에 따른 벨리보드(belly board) 하위 경계의 위치 변화에 따라 조사체적(irradiated volume) 내의 각 장기의 체적 차이 및 선량체적히스토그람을 분석하여 벨리보드의 하위 경계의 위치가 미치는 영향에 대해 알아보고자 하였다. 대상 및 방법: 2010년 5월부터 2010년 9월까지 직장암으로 수술 전이나 후에 방사선치료를 위해 벨리보드만 사용하여 전산화 단층촬영을 시행한 경우와 방광압박도구를 추가하여 전산화 단층촬영을 재 시행한 10명을 대상으로 후향적으로 분석하였다. 환자의 중앙나이는 64세(범위, 45∼75세)로 조직학적으로 선암 진단을 받고, 수술 전 혹은 후에 동시항암화학방사선요법을 시행받았다. 방광압박도구 사용 전후 각 체적과 조사체적비의 평균비교 및 복강과 골반강의 조사체적의 합과 소장의 체적비와 방광의 체적비 간의 상관관계를 분석하였다. 결 과: 방광압박도구 추가 후 벨리보드 하위

목적: 간세포암의 골전이가 증가됨에 따라 이에 대한 치료의 중요성이 증가되고 있으며 본 연구는 간세포암에서 유래한 골전이 환자에서 방사선치료의 통증 조절 효과에 대하여 분석하였다. 대상과 방법: 1991년 1월부터 2000년 6월까지 간세포암에서 유래한 골전이로 방사선치료를 받은 55명의 환자 중 계획된 방사선치료를 마친 51명(77 골전이 병소)을 대상으로 하였다. 대상환자 중 남자가 45명, 여자가 6명으로 남녀 비는 7.5 대 1 이었고 연령은 21세에서 80세였고 중앙값은 55세였다. 방사선치료는 하루 1회 선량 2.5 Gy에서 4 Gy로 총선량 12.5 Gy에서 50 Gy(중앙값 30 Gy)까지 시행되었다. 방사선치료에 대한 반응은 Wisconsin Brief Pain Questionnaire 검사로 분석하였으며 생존율 분석은 Kaplan- Meier법을 이용하였고 통계적 검증은 log rank test, Cox regression을 이용하였다. 결과: 원발 병소의 진단 당
