연구실 소개
박영수 교수의 연구실은 헬리코박터 파일로리의 항생제 내성 메커니즘과 그 영향을 중심으로 한 위암 관련 미생물학적 연구를 주도하고 있습니다. 특히 국내에서의 항생제 내성 유전자 변이(예: 23S rRNA A2143G 변이) 분석과 함께, 위암의 전구 병변인 위축성 간염 및 소화성 불변성 변화의 유병률 및 위험 인자 규명에도 기여하고 있습니다. 또한, 병변의 정확한 영상학적 스테이징을 위한 내 endoscopic 초음파와 다중층 CT의 진단 정확도 평가 연구를 병행하여, 임상적 치료 전략 수립에 기여하고 있습니다. 최근에는 대장암과 관련된 장내 미생물총의 변화와 그 기능적 영향까지 연구 영역을 확장하고 있습니다.

Clarithromycin resistance of H. pylori, which determined the efficacy of H. pylori eradication of proton pump inhibitor triple regimen, was found to be increased in a single center study. The human microbiota comprises trillions of microbes, and the relationship between cancer and microbiota is very complex. These results show that there is institutional difference of antibiotic resistance of H. pylori, explaining the institutional difference of eradication rate of H. pylori. The resistance to clarithromycin seems to be an important determinant for the eradication by proton pump inhibitor triple therapy but resistance to amoxicillin does not have any effect.
Invasive H. pylori tests, especially the CLOtest, had a lower detection rate for H. pylori in the presence of mucosal atrophy and IM, and this became more prominent in the presence of higher levels of IM and AG.
These results suggest that H. pylori infection causes alterations of mucin expression, closely related with the development of gastric atrophy with intestinal metaplasia, probably contributing to carcinogenesis.
