15This retrospective study evaluated lactate clearance (LC), measured at 6, 12, 18, and 24 hours after reperfusion, as a predictor of early allograft dysfunction (EAD) and short-term outcomes in patients receiving deceased donor liver transplantation. Of 181 transplant recipients, 44 (24.3%) developed EAD and had lower LCs than those who did not develop EAD. A receiver operating characteristic analysis showed that LC determined at 6 hours showed the highest area under curve value of 0.828 (95% con
Laparoscopic and robotic surgeries were associated with shorter TIC. This study demonstrates another benefit of minimally invasive surgery with regard to early initiation of adjuvant chemotherapy.

BACKGROUND: Although discharge summaries in patient-friendly language can enhance patient comprehension and satisfaction, they can also increase medical staff workload. Using a large language model, we developed and validated software that generates a patient-friendly discharge summary. METHODS: We developed and tested the software using 100 discharge summary documents, 50 for patients with myocardial infarction and 50 for patients treated in the Department of General Surgery. For each document,
Purpose: The aim of this study is to investigate the effect of the surgical hospitalist system on postoperative outcomes and hospital costs for surgical patients. Methods: We reviewed the medical records of 522 patients who were admitted to the divisions of colorectal and gastrointestinal surgery for operation from September to December 2017 at Severance Hospital, Yonsei University College of Medicine in Seoul, Korea. All patients were divided into 2 groups; one that was managed by surgical hosp
RATIONALE: In living-donor liver transplantation (LDLT), the right lobe graft is commonly utilized to prevent small-for-size syndrome, despite the considerable donor morbidity. Conversely, the feasibility of the left lobe graft and the right posterior section graft in smaller-sized recipients is now commonly employed with comparable outcomes to right lobe grafts. The efficacy of the right anterior section graft has rarely been reported. PATIENT CONCERNS: A 56-year-old man, a heavy alcoholic beve
The development of large language models tailored for handling patients' clinical notes is often hindered by the limited accessibility and usability of these notes due to strict privacy regulations. To address these challenges, we first create synthetic large-scale clinical notes using publicly available case reports extracted from biomedical literature. We then use these synthetic notes to train our specialized clinical large language model, Asclepius. While Asclepius is trained on synthetic da
BACKGROUND: The widespread adoption of Hospital Information Systems (HIS) has brought significant benefits in healthcare quality and workflow efficiency. However, downtimes for system maintenance are inevitable and pose a considerable challenge to continuous patient care. Existing strategies, including manual prescription methods, are no longer effective due to increasing reliance on digital systems. METHOD: This study implemented two main strategies to mitigate the impact of scheduled downtimes
Background: With the aim of improving the safety and quality of medical care of inpatients, the hospitalist system was implemented as a pilot project from September 2016 in Korea. Based on the clinical results from the pilot project, the government promoted the hospitalist system to a regular service in January 2021 with the establishment of a new fee schedule. In this study, the current status of the hospitalist system in Korea and suggestions for the future direction of the new system are desc
BACKGROUND: The shortage of doctors at night makes it difficult to execute complex tasks and deliver accurate decisions. Therefore, reducing the night shift physician's workload is essential for patient safety. This study aimed to observe the effect of daytime surgical hospitalists on the reduction in night shift physicians' workload by analyzing the volume of electronic orders generated at night for postoperative patients. METHODS: A total of 9328 hospitalized patients who underwent colorectal
As the need for medical services increases, interest in the quality improvement of healthcare and patient safety is also increasing. This study aimed to propose a surgical hospitalist or surgicalist system in Korea. Specifically, it has been suggested that the essential capacities of the surgicalist, including their understanding of surgery, wound management, emergency management, surgical nutrition, and education, may be useful in improving the quality of healthcare and patient safety. The key
In order to improve patient safety and quality of care, the hospitalist system was implemented in Korea from January 2021. As hospitalists take on new roles in hospitals, changes are also required to efficient medical environment in the hospital information systems. By designing and implementing a hospitalist support system in hospital information systems, hospitalists set the authority to access patient information, and automated fee schedule to reduce billing errors. In addition, the calculati
LSA not only provides a safe alternative but also offers a resource-efficient option for wound management compared to staplers.
As expectations for medical service have increased recently, the interest in medical quality improvement and patient safety has increased. In addition, as part of efforts to improve the training environment of the residents, the working hours of the residents are reduced which means that the medical workforce is expected to change significantly in the future. As a result, in hospital care of surgical patients, it is inevitable to change the patient care system by surgeon-residents to the new sys
